摘要
目的 :将重组人骨形态发生蛋白 2 (rhBMP 2 )植入犬的自体气管移植段内 ,促进新生软骨形成 ,克服移植后软骨软化吸收造成的移植段气管塌陷。 方法 :2 4只杂种犬随机等分为三组 ,颈部切除 5个气管环段作为移植段。A组原位再植 ,外覆带蒂大网膜 ;B组移植段植入rhBMP 2后埋入腹腔大网膜中 ;C组移植段植入rhBMP 2后原位再植 ,外覆带蒂大网膜。以上rhBMP 2均以胶原溶液作为缓释载体 ,用注射器直接注入各环间。术后 1、2、4、8周处死动物 ,标本行大体及组织学观察。 结果 :三组移植体软骨环均有不同程度的吸收。B、C组移植体的rhBMP 2植入区有软骨再生 ,且以B组再生更明显。 结论 :rhBMP 2可刺激气管移植段软骨再生 ;胶原可充当rhBMP 2的缓释载体 。
Objectives: One of the serious problems in tracheal transplantation is severe stenosis of the graft caused by chondrolysis. To prevent collapse of the trachea following softening and absorption of the cartilage after transplantation, recombinant human bone morphogenetic protein 2 (rhBMP 2) was implanted into the tracheal autograft to induce regeneration of dog's tracheal cartilage. Methods:Twenty four mongrel dogs were randomly and equally divided into group A ,B and C. A five ring cervical tracheal segment was harvested as autograft. In group A, the graft was reimplanted and wrapped with pedicled omentum. In group B, the graft was heterotopically transplanted into the omentum, after which, rhBMP 2 with collagen as carrier was injected into the soft tissue between cartilaginous rings of the autograft. In group C, after injection of rhBMP 2, the graft was reimplanted with omentopexy as in group A. The animals were sacrificed on the 1, 2, 4 and 8 weeks postoperatively, and the postmortem specimens were examined grossly and histologically. Results: Cartilage rings were absorbed in all the three groups in various degrees. In group B and C, new cartilage was induced in the BMP injected area and more new cartilage was observed in group B than in group C. Conclusions:The results indicated that rhBMP 2 could induce new cartilage formation in the tracheal autograft. The collagen was effective as a carrier of rhBMP 2 and could enhance cartilage regeneration by rhBMP 2 after tracheal transplantation.
