摘要
目的 探讨中、晚期肺癌经介入化疗加放疗的疗效。方法 采用经支气管动脉灌注化疗二次 ,第二次动脉灌注化疗后一周加直线加速器外照射放疗 ,治疗不能手术切除的 B+ 期非小细胞肺癌 6 9例 ,放疗剂量 5 5～6 6 Gy。与同期单纯放疗的中、晚期非小细胞肺癌 6 5例疗效对比。结果 介入加放疗组 :CR2 4 .6 %、PR6 0 .9%、总缓解率 (CR+PR) 85 .5 %。单纯放疗组 :CR 2 1.5 %、PR 4 9.3%、总缓解率 (CR+PR) 70 .8%。 1、2和 3年生存率 :介入加放疗组分别为 71.0 % (49/6 9)、2 7.5 % (19/6 9)和 13.0 % (9/6 9) ;单纯放疗组 5 3.8% (35 /6 5 )、2 1.5 % (14 /6 5 )和10 .8% (7/6 5 )。结论 介入化疗加放疗治疗中晚期肺癌近期疗效和 1年生存率明显提高 ,但对 2。
Objective To study the effect of bronchial artery infusion chemotherapy in combination with radiotherapy in treatment of patients with midterm advanced lung cancer. Methods Sixty nine patients without surgical stage Ⅲ B+Ⅳ non small cell lung cancer received bronchial artery infusion chemotherapy plus radiotherapy with medical linear accelerator. A week after the second interventional chemotherapy into bronchial artery, they received radiotherapy with doses of 55～66 Gy/28~33 fx in 45(37~49) days at 2.0 Gy per fraction. A comparative study was done between the group of 69 patients treated by bronchial artery infusion plus radiotherapy and the group of 65 patients treated by radiotherapy only at the same time. Results The overall response rate was 85.5%(59/69) with CR of 24.6%(17/69) and PR of 60.9%(42/69) in group with interventional chemotherapy plus radiotherapy and that was 70.8%(46/65) with CR of 21.5%(14/65) and PR of 49.3%(32/65) in group with radiotherapy only. The difference of the two groups in CR and PR was significant ( P < 0.05 ). The 1 ,2 and 3 year survival rates of interventional chemotherapy plus radiotherapy were 71.0%(49/69),27.5%(19/69), and 13.0% (9/69) respectively and those of radiotherapy only were 53.8%(35/65),21.5%(14/65), and 0.8%(7/65) respectively. There was significant difference of 1 year survival rate between two groups ( P <0.05). Conclusion Bronchial artery infusion in combination with radiotherapy can improve the efficacy and raise the 1 year survival rate of patients with midterm advanced lung cancer.
