摘要
目的分析主动脉球囊反搏(IABP)联合参附注射液治疗急性心肌梗死(AMI)心源性休克(CS)患者的效果。方法选取2018年8月至2023年8月在我院重症监护病房(ICU)收治的60例AMI合并CS患者作为研究对象,根据治疗方法将采用IABP治疗的30例患者列为对照组,将采用参附注射液+IABP治疗的30例患者列为研究组。比较2组中医证候积分、心泵功能[心脏指数(CI)、左室射血分数(LVEF)、每搏输出量(SV)]、组织灌注氧代谢(乳酸、中心静脉血氧饱和度、尿量)。结果研究组治疗后中医证候积分低于对照组(P<0.05);研究组治疗后LVEF、CI、SV高于对照组(P<0.05);研究组治疗后乳酸低于对照组,中心静脉血氧饱和度、尿量高于对照组(P<0.05)。结论参附注射液联合IABP治疗AMI并CS效果显著,可减轻患者症状,改善组织灌注氧代谢,改善心泵功能,稳定病情。
Objective To analyze the effect of intra-aortic balloon pump(IABP)combined with Shenfu injection in the treatment of cardiogenic shock(CS)in patients with acute myocardial infarction(AMI).Methods A total of 60 patients with cardiogenic shock caused by AMI combined with CS admitted to the ICU of our hospital from August 2018 to August 2023 were selected as the research objects.Among them,30 patients treated with IABP were listed as a control group,and 30 patients treated with Shenfu injection+IABP were listed as a study group according to the treatment methods.The TCM syndrome score,cardiac pump function[stroke volume(SV),left ventricular ejection fraction(LVEF),cardiac index(CI)],tissue perfusion oxygen metabolism(lactic acid,central venous oxygen saturation,urine volume)were compared between the two groups.Results The TCM syndrome score of the study group was lower than that of the control group after treatment(P<0.05),LVEF,CI and SV in the study group were higher than those in the control group after treatment(P<0.05).After treatment,the central venous oxygen saturation and urine volume of the study group were higher than those of the control group,and the lactic acid was lower than that of the control group(P<0.05).Conclusion The effect of Shenfu injection combined with IABP in the treatment of AMI complicated with CS is significant,which can relieve the symptoms of patients,improve tissue perfusion oxygen metabolism,improve cardiac pump function,and stabilize the disease.
