冻胚移植前子宫内膜准备方案的研究进展 被引量:1

摘要 自世界上第1例试管婴儿诞生以来,辅助生殖技术迅猛发展。冻胚移植(FET)可以实现胚胎利用的最大化,提高累积妊娠率,且降低卵巢过度刺激综合征的发生风险,已广泛应用于临床。除了胚胎自身质量外,子宫内膜容受性、胚胎与内膜的同步性以及合适的内膜准备方案均是影响FET成功与否的关键。越来越多的研究表明子宫内膜准备与妊娠结局及其并发症之间的相关性,临床上往往根据患者情况进行个体化选择。目前常用的子宫内膜准备方案包括自然周期、促排卵周期、人工周期(包括激素替代周期和促性腺激素释放激素激动剂降调节联合激素替代周期),本文总结不同子宫内膜准备方案的特点、适用人群及围生期结局,以期为指导临床实践提供参考。 Since the birth of the first test-tube baby in the world,assisted reproductive technology has developed rapidly.frozen-thawed embryo transfer(FET)can maximize embryo utilization,increase cumulative pregnancy rate,and reduce the risk of ovarian hyperstimulation syndrome,which has been widely used in clinical practice.In addition to the quality of the embryo itself,endometrial receptivity,synchronization between the embryo and endometrium,and appropriate endometrial preparation plan are all key factors affecting the success of FET.An increasing number of studies have shown a correlation between endometrial preparation and pregnancy outcome and its complications,and personalized selection is often made based on the patient’s condition in clinical practice.The commonly used endometrial preparation methods currently include natural cycle,ovarian stimulation,and artificial cycle(including hormone replacement therapy and gonadotropin releasing hormone-agonist-a down-regulation in combination with hormone replacement therapy).This paper summarizes the characteristics,applicable populations,and perinatal outcome of different endometrial preparation methods,in order to provide reference for guiding clinical practice.
