摘要
目的 探讨手指甲下血管球瘤的发病因素、影像学特征及治疗体会.方法 我科自2015年10月至2023年3月收治手指甲下血管球瘤患者35例36指,根据病史、症状、体征,应用Love试验、冷敏感试验、Hildreth试验、透光试验等特异性检查,结合高频超声、X线等影像学检查,对指甲下肿瘤精准定位、定量.35例36指均应用显微外科技术行甲下血管球瘤切除术.结果 术后患者切口均Ⅰ期愈合,病理检查证实为血管球瘤.术后随访9~24个月,平均12个月,35例36指均治愈,疼痛症状消失,无复发病例.结论 甲下血管球瘤具有特征性的临床表现,结合影像学检查有助于术前定位和明确诊断,手术切除肿瘤尤其是显微外科切除是治疗血管球瘤的有效方法.
Objective To investigate the pathogenic factors,imaging characteristics,and treatment experience of subungual glomus tumor.Methods From October 2015 to March 2023,35 patients(36 fingers)with subungual glomus tumor were treated.Based on their medical history,symptoms,and physical signs,specific examinations such as Love test,cold sensitivity test,Hildreth test,and translucency test were performed,combined with high-frequency ultrasound,X-ray and other imaging examinations,subungual glomus tumor was accurately located and quantified.35 cases(36 fingers)were all treated with microsurgical techniques for the resection of subungual glomus tumor.Results The incisions of all patients achieved primary healing postoperatively,and pathological examination confirmed the presence of glomus tumor.The postoperative follow-up lasted for 9 to 24 months,with an average of 12 months.All 35 cases(36 fingers)were cured,and pain symptoms disappeared without recurrence.Conclusion The subungual glomus tumor has characteristic clinical manifestations,and combined with imaging examination,it is helpful for preoperative localization and clear diagnosis.The surgical resection of the tumor,especially microsurgical resection,is an effective method for treating glomus tumor.
