摘要
目的调研糖皮质激素类药物在江西省医疗机构中儿科神经系统疾病的临床应用,为促进临床合理用药提供思路与依据。方法依托江西省药物警戒专业委员会平台,选取省内具备儿科的医疗机构作为研究对象,以调研问卷形式向各医疗机构收集数据,从多个维度对儿科神经系统疾病使用糖皮质激素情况开展调查,并对相关数据进行描述性分析。结果调研了江西省51家医疗机构的18种儿科神经系统疾病治疗用药情况,共有不同规格的注射用糖皮质激素类药物13种。给药途径以静脉滴注/推注为主,占比为94.12%。最常见用于治疗的儿科神经系统疾病为热性惊厥、急性中毒性脑病、化脓性脑膜炎和病毒性脑炎。近半数疾病的使用疗程无明确标准,具有明确使用指南医疗机构占比为43.14%~56.86%,全省各地区医疗机构均普遍配备糖皮质激素类药物用于治疗儿科神经系统疾病,但存在部分医疗机构使用不规范的情况。结论糖皮质激素在儿科神经系统疾病的临床应用有待进一步完善,以期通过制定相关专家共识或指南,进行培训和推广,提升合理用药管理水平,加强用药监护,搭建使用数据共享平台,促进糖皮质激素在儿科神经系统疾病的合理使用。
Objective To investigate the clinical application of glucocorticoids drugs in pediatric nervous system diseases in medical institutions in Jiangxi Province,and to provide ideas and basis for promoting clinical rational drug use.Methods Relying on the platform of pharmacovigilance Committee of Jiangxi Province,medical institutions with pediatrics in Jiangxi Province were selected as the research objects.Data were collected from medical institutions in the form of questionnaires to investigate the use of glucocorticoids in pediatric neurological diseases from multiple dimensions,and the relevant data were analyzed descriptively.Results Investigated the use of 18 kinds of pediatric nervous system diseases in 51 medical institutions in Jiangxi Province.There were 13 kinds of glucocorticoids for injection of different specifications.The main route of administration was intravenous infusion/push injection,accounting for 94.12%.The most common pediatric neurological diseases used for treatment were febrile convulsion,acute toxic encephalopathy,suppurative meningitis and viral encephalitis.The treatment course of nearly half of the diseases had no clear standard,and the proportion of medical institutions with clear guidelines for use was 43.14%-56.86%.Medical institutions in all regions of the province are generally equipped with glucocorticoids for the treatment of pediatric neurological diseases,but some medical institutions were not standardized.Conclusion The clinical application of glucocorticoids in pediatric nervous system diseases needs to be further improved,with a view to promoting the rational use of glucocorticoids in pediatric nervous system diseases through the formulation of relevant expert consensus or guidelines,training and promotion,improving the level of rational drug use management,strengthening medication monitoring,and building a data sharing platform.
