摘要
中医药在治疗慢性肾脏病(chronic kidney disease,CKD)中具有独特优势,可将CKD的核心病机概括为“真邪相搏,虚实夹杂”。中医理论正邪发病观与肾脏疾病中现代免疫学的发病机制密切相关,“正气”是维持或调节免疫动态平衡的体现,“邪气”则代表诱发免疫异常的病理因素或因免疫功能异常形成的对机体有害的病理产物。基于“真邪相搏,虚实夹杂”的病机特点,运用中医药扶正祛邪法可改善由直接或间接免疫介导的肾脏损害,纠正免疫紊乱,改善肾脏病理表现及临床症状,起到肾脏保护的作用。文章对扶正祛邪法在CKD治疗中的理论依据、基础研究和临床应用进行综述,以期为中医药治疗CKD提供依据。
Chinese medicine has unique advantages in the treatment of chronic kidney disease(CKD),and the core pathogenesis of CKD can be summarized as“struggling between healthy Qi and pathogens,intermingling between deficiency and excess”.The concept of healthy Qi and pathogentic factors in traditional Chinese medicine theory is closely related to the pathogenesis of modern immunology in kidney diseases.The“healthy Qi”is the embodiment of maintaining or regulating the dynamic balance of immunity,while the“pathogenic Qi”represents the pathological factors that induce immune abnormalities or the pathological products that are harmful to the body due to abnormal immune function.Based on the pathogenesis characteristics of“struggling between healthy Qi and pathogens,intermingling between deficiency and excess”,the use of traditional Chinese medicine to strengthen the“healthy Qi”and eliminate pathogenic factors can improve kidney damage mediated by direct or indirect immunity,correct immune disorders,improve renal pathological manifestations and clinical symptoms,and play a role in in renal protection.The article summarizes the theoretical basis,basic research and clinical application of the therapeutic principle of strengthening the“healthy Qi”and eliminating the pathogenic factor in the treatment of CKD,in order to provide basis for the treatment of CKD with traditional Chinese medicine.
