摘要
目的:分析基于细胞因子肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)变化研究小续命汤加减治疗痛性糖尿病周围神经病变(PDPN)阳虚寒凝证的疗效。方法:将102例PDPN患者按照随机数字表法进行分组,对照组(n=51)及治疗组(n=51),对照组实施常规治疗,观察组在上述基础联合小续命汤加减治疗,比较两组临床疗效、Toronto临床神经病变评分(TCSS)、疼痛情况、神经传导速度及血清TNF-α、IL-6水平,统计不良反应发生情况。结果:治疗组临床总有效率(94.12%)较对照组(72.55%)明显高(P<0.05);治疗8周后,治疗组TCSS评分、疼痛视觉模拟量表(VAS)评分均相较于对照组更低(P<0.05);治疗8周后,治疗组周围神经传导速度相较于对照组更高(P<0.05);治疗8周后,治疗组血清TNF-α、IL-6水平相较于对照组更低(P<0.05);两组患者未出现任何不良反应。结论:小续命汤加减治疗阳虚寒凝证PDNP,疗效明显,可改善患者临床症状,缓解疼痛,增加周围神经传导速度,且能下调TNF-α、IL-6水平。
Objective:To analyze the effect of modified Xiaoxuming decoction in the treatment of painful diabetic peripheral neuropathy(PDPN)with yang deficiency and cold coagulation syndrome based on the changes in tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6).Methods:102 patients with PDPN and yang deficiency and cold coagulation syndrome were divided into the control group(n=51)and the treatment group(n=51).The control group was given conventional treatment,while the observation group was treated with modified Xiaoxuming decoction on above basis.The two groups were compared in terms of clinical efficacy,Toronto Clinical Scoring System(TCSS)score of neuropathy,pain degree,nerve conduction velocity,and serum TNF-αand IL-6 levels.The incidence rates of adverse reactions were calculated.Results:The total clinical effective rate of the treatment group(94.12%)was significantly higher than that of the control group(72.55%)(P<0.05).After 8 weeks of treatment,comparison between groups found that TCSS score and Visual Analogue Scale(VAS)score of pain in the treatment group were lower(P<0.05).Peripheral nerve conduction velocity in the treatment group was higher(P<0.05).Serum TNF-αand IL-6 levels in the treatment group were lower(P<0.05).No adverse reactions were observed in either group.Conclusion:Modified Xiaoxuming decoction can achieve significant therapeutic effect on PDNP with yang deficiency and cold coagulation syndrome.It can improve the patients’clinical symptoms,relieve pain,increase peripheral nerve conduction velocity,and downregulate TNF-αand IL-6 levels.
