摘要
[目的]分析甘肃省肺癌患者住院费用构成及影响因素。[方法]收集甘肃省公立医院2013—2022年就诊的肺癌患者病案首页数据,采用描述性统计方法对患者人口学特征和费用构成进行统计描述。应用多重线性回归、分位数回归分析住院费用的影响因素。[结果]共纳入43674例甘肃省肺癌患者,住院总费用共计518997144.36元,患者住院费用中位数(下四分位数,上四分位数)为7313.48(4376.86,12372.75)元,其中药品费(185159061.00元)、诊断费(134555304.10元)、治疗费(98775029.40元)为排名前3的费用类型。多重线性回归分析结果显示,不同性别、年龄、地区经济发展情况、入院途径、住院天数、医疗付款方式对患者住院费用的影响均有统计学意义(P<0.05)。分位数回归模型结果显示,地区经济发展情况、入院途径、医疗付款方式和住院天数在住院费用三个分位数点上均产生了较为显著的影响(P<0.05)。[结论]甘肃省肺癌患者经济负担较重,住院费用构成有待优化。应加强肺癌防治宣传工作,减少无效住院日,扩大医保覆盖范围,降低肺癌患者的住院费用。
[Purpose]To analyze the composition of hospitalization expenses and influencing factors of lung cancer patients in Gansu Province.[Methods]The data of lung cancer patients admitted to public hospitals in Gansu Province from 2013 to 2022 were collected from their medical records,and the demographic characteristics and expenses composition of patients were analyzed.The multiple linear regression and quantile regression were applied to analyze the influencing factors of hospitalization expenses.[Results]A total of 43674 lung cancer patients were included in this study,and the total hospitalization expenses of the patients were 518997144.36 CNY,of which the drug expenses(185159061.00 CNY),the diagnosis expenses(134555304.10 CNY)and the treatment expenses(98775029.40 CNY)were the top 3 expense types;and the median(P25,P75)hospitalization expenses per admission was 7313.48(4376.86,12372.75)CNY.The results of multiple linear regression analyses showed that the sex,age,area,admission mode,length of hospital stay and payment methods were significantly associated with hospitalization expenses(P<0.05).The results of the quantile regression model showed that area,admission mode,payment methods,and length of hospital stay had a significant effect at all three quantile points of hospitalization expenses(P<0.05).[Conclusion]The composition of hospitalization expenses of lung cancer patients in Gansu Province needs to be optimized,and necessary measures should be taken according to the influencing factors.
