直线偏振光近红外线照射对背根神经节脉冲射频治疗带状疱疹后神经痛患者效果的影响 被引量:1

Effects of linear polarized light near infrared irradiation on pulsed radiofrequency of dorsal root ganglion in patients with postherpetic neuralgia

摘要 目的评价直线偏振光近红外线照射(SL)对背根神经节脉冲射频(PRF)治疗带状疱疹后神经痛(PHN)效果的影响。方法回顾性收集2022年1月1日至2022年12月31日在天津医科大学第二医院疼痛治疗中心就诊的PHN患者,年龄≥18岁,视觉模拟评分(VAS)≥4分。按照治疗方式不同分为2组,直线偏振光近红外线照射联合背根神经节脉冲射频组(SL+PRF组,n=42)和单纯背根神经节脉冲射频组(PRF组,n=48)。PRF参数为45 V、2 Hz、20 ms、42℃、120 s,2次;SL照射探头距离皮肤50~60 cm,输出功率量为100%,15 min/次。收集所有患者的一般资料,并记录入院时、术前及术后1 d、3 d、7 d、14 d、1个月和3个月时的VAS评分、匹兹堡睡眠质量指数(PSQI)及病损皮肤与正常皮肤的温度差,并进行统计学分析。结果VAS评分存在显著的组间效应(Waldχ^(2)=14.00,P<0.001),时间效应(Waldχ^(2)=858.96,P<0.001)和交互效应(Waldχ^(2)=18.52,P=0.010);SL+PRF组术前及术后14 d、28 d、3个月的VAS评分均低于PRF组(P均<0.05)。PSQI评分存在显著的组间效应(Waldχ^(2)=32.69,P<0.001),时间效应(Waldχ^(2)=2436.36,P<0.001)和交互效应(Waldχ^(2)=68.75,P=0.010);SL+PRF组术前及术后7 d、14 d、28 d、3个月的PSQI评分均低于PRF组(P均<0.05)。SL+PRF组术后14 d及3个月的皮损温度差均显著低于PRF组(P均<0.05)。结论SL可改善PRF对PHN的治疗效果,在降低疼痛程度,改善睡眠质量及促进皮损康复方面表现出积极影响。 Objective To investigate the effects of linear polarized light near infrared irradiation(Superlizer,SL)on pulsed radiofrequency(PRF)of dorsal root ganglion in patients with postherpetic neuralgia(PHN).Methods PHN patients in the Pain Management Center,Second Hospital of Tianjin Medical University,from January 1st 2022 to December 31st 2022,aged≥18 years old,visual analogue score(VAS)≥4,were retrospectively selected.According to the different treatment methods,they were divided into two groups:linear polarized light near infrared irradiation combined with pulsed radiofrequency of dorsal root ganglion group(SL+PRF group,n=42)and simple pulsed radiofrequency of dorsal root ganglion group(PRF group,n=48).The PRF parameters were 45 V,2 Hz,20 ms,42℃,120 s,twice.The distance was 50-60 cm between the SL irradiation probe and the skin,and the output power was 100%,focus 15 min each time.The general data were collected,and VAS,Pittsburgh sleep quality index(PSQI)and the temperature difference between diseased skin and normal skin were recorded at admission,before the treatment and 1 day,3 days,7 days,14 days,1 month and 3 months after the treatment.And statistical analysis was carried out.Results VAS had significant intergroup effects(Waldχ^(2)=14.00,P<0.001),time effects(Waldχ^(2)=858.96,P<0.001)and interaction effects(Waldχ^(2)=18.52,P=0.010);and VAS was lower in the SL+PRF group than that in the PRF group before the treatment and 14 days,28 days and 3 months after the treatment(all P<0.05).PSQI had significant intergroup effects(Waldχ^(2)=32.69,P<0.001),time effects(Waldχ^(2)=2436.36,P<0.001)and interaction effects(Waldχ^(2)=68.75,P=0.010);and PSQI was lower in SL+PRF group than that in PRF group before the treatment and 7 days,14 days,28 days and 3 months after the treatment(all P<0.05).The temperature difference of skin lesions was significantly lower in SL+PRF group than that in PRF group at 14 days and 3 months after the treatment(all P<0.05).Conclusion SL can improve the efficacy of PRF for patients with PHN,showing positive effects in reducing the degree of pain,improving sleep quality and promoting recovery of the skin lesions.
