摘要
目的了解上海市内120急救医师群体对急危重症患者的转运情况,分析其中存在的问题,并提出相应改进措施.方法利用调查问卷,收集2022年11月1日至2023年11月14日全市10家120急救中心的399名急救医师参与的急危重症患者院间转运情况,并对结果进行统计分析.结果急危重症患者转运决策制定有一半决策由患者及家属主动决定,患者知情同意书上存在对病情的不了解,转运前的准备工作、风险评估、不良事件的应急管理存在相应的不足.虽然人员设备的配置、途中监测及转运交接基本满足需求,但仍存在一定问题.结论从医护人员角度了解我市急危重症患者转运情况,有利于促进急危重患者转运模式流程的规范化、标准化及应急管理的及时性、完整性,提升我国急危重症患者院间转运能力.
Objective To understand the status of transportation of emergency and critical patients of “120” emergency physicians in Shanghai, analyze the existing problems, and propose corresponding improvement measures. Methods A total of 399 emergency physicians from 10 “120” first-aid centers participated in the interhospital transportation of critical patients from November 1st , 2022 to November 14th , 2023, were collected, and the results were statistically analyzed. Results Half of the transport decision-making for acute and critical patients was made by patients and their family members. There was a lack of understanding about the medical condition on the patient's informed consent form. Also, there were some problems in the preparation, risk assessment, and emergency management of adverse events before transshipment. The configuration of personnel and equipment, on-route monitoring, and transshipment can basically met the requirements, but there were still some problems. Conclusion Understanding the status of transportation of emergency and critical patients in our city from the perspective of medical and nursing personnel is beneficial to promote the standardization of transportation mode of the flow of emergency and critical patients and improve the timeliness and integrity of emergency management, thus improve the inter-hospital transfer capacity of critical patients in our country.
