摘要
目的 探讨动脉夹层动脉瘤患者在围手术期采用循证护理结合舒适护理的效果。方法 选择2021年8月至2022年8月前往厦门市第一医院就诊的82例动脉夹层动脉瘤患者为研究对象,根据随机数字表法将其分为对照组和观察组,每组41例。对照组给予常规护理,观察组实施循证护理结合舒适护理。比较2组的血压[收缩压(SBP)、舒张压(DBP)]、心功能指标[左室舒张末期内径(LVEDD)和左室射血分数(LVEF)]、生活质量和术后并发症发生率。结果 术后3 d, 2组的SBP和DBP均较术前降低(P均<0.05),且观察组的SBP和DBP均低于对照组(P均<0.05)。术后3 d和出院时,2组的LVEDD均小于入院时,LVEF均低于入院时(P均<0.05),且观察组的LVEDD均小于同期对照组,LVEF均低于同期对照组(P均<0.05)。术后3 d和出院时,2组的躯体功能、心理状态、社会交往和自护能力评分均高于入院时(P均<0.05),且观察组均高于同期对照组(P均<0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。结论 循证护理结合舒适护理有助于稳定动脉夹层动脉瘤患者在手术前后的血压,改善其心功能指标和生活质量,降低并发症发生率。
Objective To explore the application effect of evidence-based nursing combined with comfort care in patients with arterial dissection aneurysm during the perioperative period.Methods A total of 82 patients with arterial dissection aneurysm who went to the First Affiliated Hospital of Xiamen University from August 2021 to August 2022 were selected as the study subjects,and were divided into a control group and an observation group according to the random number table method,with 41 cases in each group.The control group was given routine nursing,and the observation group was given evidence-based nursing combined with comfort care.Blood pressure(systolic blood pressure[SBP],diastolic blood pressure[DBP]),cardiac function indexes(left ventricular end-diastolic diameter[LVEDD]and left ventricular ejection fraction[LVEF]),quality of life and incidence of postoperative complications were compared between the two groups.Results On the 3rd day after surgery,the SBP and DBP of both groups decreased compared to before surgery(both P<0.05),and the SBP and DBP in the observation group were lower than those in the control group(both P<0.05).On the 3rd day after surgery and at discharge,the LVEDD and LVEF in both groups were lower than those at admission(both P<0.05),and the LVEDD and LVEF in the observation group were lower than those in the control group during the same period(both P<0.05).On the 3rd day after surgery and at discharge,the scores of physical function,psychological state,social interaction and self-care ability of patients in both groups were higher than those at admission(all P<0.05),and those scores in the observation group were higher than those in the control group during the same period(all P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion Evidence-based nursing combined with comfort care can help stabilize the blood pressure of patients with arterial dissection aneurysm before and after surgery,improve the cardiac function indexes and quality of life,and reduce the incidence of complications.
