摘要
目的 比较自动调节持续气道正压通气(AutoCPAP)与双水平气道正压通气(Bi PAP)对改善阻塞性睡眠呼吸暂停低通气综合征(OSAHS)睡眠、认知及心境状态的改善作用。方法 选取2021年6月~2022年6月于浙江省绍兴市第七人民医院就诊的OSAHS患者102例,随机分成两组,分别给予为期4周AutoCPAP与BiPAP治疗。比较治疗前后两组睡眠、认知与心境状态相关指标的差异。结果 干预治疗前,组间所有研究指标差异均无统计学意义(P>0.05);治疗后,睡眠相关指标方面,AutoCPAP组SL、SE、微觉醒指数、REM/TST、PSQI评分以及ESS评分优于BiPAP组,差异有统计学意义(P<0.05);认知方面,AutoCPAP组MoCA量表亚维度“视空间与执行”和“注意评分”优于BiPAP组,差异有统计学意义(P<0.05);心境状态方面,AutoCPAP组POMS量表亚维度“紧张-焦虑”和“疲乏-迟钝”评分优于BiPAP组,差异有统计学意义(P<0.05)。结论 AutoCPAP较BiPAP对OSAHS患者睡眠质量提高、认知与心境状态改善均有更显著的治疗效果。
Objective To compare the therapeutic effects of AutoCPAP and BiPAP on improving sleep,cognition and mood status in patients with OSAHS.Methods From June 2021 to June 2022,102 patients of Zhejiang Shaoxing Sevenh People’s Hospital with OSAHS were randomly divided into two groups and treated with AutoCPAP and BiPAP for 4 weeks respectively.The differences of sleep,cognition and mood state related indexes between groups before and after treatment were compared.Results Before intervention,there was no significant difference in all research indicators between the two groups(P>0.05).After treatment,in terms of sleep-related indicators,SL、SE、microarousal index、REM/TST、PSQI score and ESS score in AutoCPAP group were better than those in BiPAP group(P<0.05).In terms of cognition,the scores of sub-dimensions visual space and execution and attention of MoCA scale in AutoCPAP group were better than those in BiPAP group(P<0.05).In terms of mood state,the POMS sub-dimension scores of tension-anxiety and fatigue-impairment in AutoCPAP group were relatively better(P<0.05).Conclusion AutoCPAP is more effective than BiPAP in improving sleep quality、cognition and mood of OSAHS patients.
