摘要
目的探讨微创拔牙术在下颌阻生智齿中的应用价值。方法选取2020年2月—2022年4月无锡口腔医院收治的66例下颌阻生智齿患者为研究对象,按投掷硬币法分为参照组和研究组,各33例。参照组施以传统拔牙术治疗,研究组施以微创拔牙术治疗。对比两组患者治疗总有效率、视觉模拟评分法(Visual Analogue Scale,VAS)评分、术后肿胀程度、手术时间以及并发症(术后感染、牙龈撕裂、干槽症、张口受限、下牙槽神经损伤)总发生率。结果研究组治疗总有效率为96.97%,高于参照组,差异有统计学意义(χ^(2)=6.304,P<0.05)。研究组患者治疗后1、3 dVAS评分低于参照组,差异有统计学意义(P<0.05)。研究组患者术后肿胀程度低于参照组,手术时间短于参照组,差异有统计学意义(P<0.05)。研究组患者并发症总发生率低于参照组,差异有统计学意义(P<0.05)。结论临床对微创拔牙术患者合理采用口腔颌面锥形束CT展开配合治疗,对于疗效以及安全性提高,疼痛程度以及术后肿胀程度缓解,手术时间的缩短,可以获得显著效果。
Objective To explore the application value of minimally invasive tooth extraction in the treatment of mandibular impacted wisdom teeth.Methods 66 patients with mandibular impacted wisdom teeth admitted to Wuxi Stomatological Hospital from February 2020 to April 2022 were selected as the study objects.They were divided into the reference group and the research group,according to the coin tossing method with 33 cases in each group.The patients of the reference group were treated with traditional tooth extraction,while the patients of the research group were treated with minimally invasive tooth extraction.The total treatment effective rate,Visual Analogue Scale(VAS)scores,postoperative swelling,operation time,and total incidence of complications(postoperative infection,gum tears,dry socket,limited mouth opening,inferior alveolar nerve injury)between the two groups of patients were compared.Results The total effective rate in the research group was 96.97%,which was higher than that of the reference group,and the difference was statistically significant(χ^(2)=6.304,P<0.05).The 1,3 days VAS scores after treatment in the research group were higher than those of the reference group,and the difference was statistically significant(P<0.05).The postoperative swelling of patients in the research group was better than that of the reference group,and the operation time was shorter than that of the reference group,and the difference was statistically significant(P<0.05).The total incidence rate of complications in the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).Conclusion The reasonable use of oral and maxillofacial cone beam CT for clinical minimally invasive tooth extraction patients can achieve significant results in improving efficacy and safety,alleviating pain and postoperative swelling,and shortening operation time.
