The Association Between Insulin Resistance and Lipid Metabolism in Type Ⅱ Diabetes Mellitus

摘要 目的 研究Ⅱ型糖尿病患者胰岛素抵抗与血脂代谢的关系。方法 测定109例糖尿病患者及86例正常人空腹血糖(FPG)、糖化血红蛋白(HbA_(1c))、空腹胰岛索(FINS)、体重指数(BMI)、腰臀比(WHR)及脂代谢指标,并进行相关性分析。结果 (1)109例糖尿病患者BMI、FPG、HbA_(1c)、FINS、胆固醇(CHO)、甘油三酯(TG)、极低密度脂蛋白(VLDL)、载脂蛋白B(ApoB)及胰岛素抵抗指数(HOMA-IR)明显高于正常对照组。(2)Pearson相关分析表明,HOMA-IR与BMI、腰/臀、TG、VLDL呈正相关。(3)逐步回归分析发现BMI和TG对HOMA-IR有影响。以HOMA-IR为因变量,BMI、TG为自变量,发现BMI对HOMA-IR有影响。结论 高TG是Ⅱ型糖尿病胰岛素抵抗的特点,BMI升高可作为Ⅱ型糖尿病胰岛素抵抗的独立因素。 OBJECTIVE To study the association between insulin resistance and lipid metabolism in type H diabetes mellitus(DM). METHOD Fasting plasma glucose(FPG).,HbA1c.Fasting insulin (FINS)., body mass index (BMI). waist hip ratio(WHR) and plasma lipids were determined in 109 case of type Ⅱ diabetes and 86 case of health controls. RESULTS (1)The levels of BMI.FPG,HbA1C.FINS.triglyceride(TG), cholesterol (CHO) .very low density lipoprotein(VLDL) ,apolipoproteinB(ApoB) and homeostasis model assessment-insulin resistance (HOMA-IR) in type Ⅱ DM were higher than those in the control group. (2)Pearson correlation analysis indicated that therebe existed positive correlations between HOMA - IR and BMI.WHR.TG.VLDL. (3)Stepwise regression analysis in dicated that BMI and TG were associated with HOMA - IR. And HOMA - IR was independently associated with BMI. CONCLUSIONS Type II diabetes patients with insulin resistance are characterized by high TG levels,and BMI could be regarded as independent factors for insulin resistance.
