摘要
目的系统评价针灸治疗帕金森病合并吞咽障碍的有效性和安全性。方法检索知网(CNKI)、万方、维普、中国生物医学文献数据库(CBM)、PubMed、The Cochrane Library、Embase、Clinical trials、Web of science数据库,检索时间为建库至2022年8月。筛选与针灸防治帕金森病合并吞咽障碍有关的论文。结果最终纳入9个随机对照试验(RCT),610例患者。meta分析结果显示:试验组治疗帕金森病合并吞咽障碍的总有效率、标准吞咽功能评估量表(SSA)评分、吞咽透视检查(VFSS)评分及营养学指标均优于对照组[RR=1.37,95%CI(1.24,1.50),P<0.00001]、[MD=-2.62,95%CI(-3.95,-1.29),P=0.0001]、[MD=1.30,95%CI(0.88,1.73),P<0.00001]、[SMD=1.16,95%CI(0.37,1.96),P=0.004]。结论针灸治疗可以改善帕金森病合并吞咽障碍患者的吞咽功能及营养情况,但本结论仍需要更高质量的研究加以验证。
Objective To systematically evaluate the efficacy and safety of acupuncture in the treatment of Parkinson’s disease combined with dysphagia.Methods Papers related to acupuncture for the prevention and treatment of Parkinson’s disease combined with dysphagia were screened by searching China national knowledge infrastructure(CNKI),Wanfang,VIP,China biology medicine(CBM),PubMed,The Cochrane Library,Embase,Clinical trials,and Web of Science databases from the time of database construction to August 2022.Results Nine randomized controlled trials involving 610 patients were eventually included.Meta-analysis revealed that the total effective rate(RR=1.37,95%CI[1.24,1.50],P<0.00001),standardized swallowing assessment(SSA)scores(MD=-2.62,95%CI[-3.95,-1.29],P=0.0001),videofluoroscopic swallowing study(VFSS)scores(MD=1.30,95%CI[0.88,1.73],P<0.00001)and nutritional indicators(SMD=1.16,95%CI[0.37,1.96],P=0.004)for the treatment of Parkinson’s disease combined with dysphagia were superior in the experimental group to the control group.Conclusion Acupuncture treatment can improve swallowing function and nutrition in patients with Parkinson’s disease combined with dysphagia,but the present findings need to be validated in higher-quality studies.
