摘要
目的 探究早期腹腔微创引流联合连续性血液净化治疗重症急性胰腺炎患者的效果。方法 选取我院2018年10月至2020年10月收治的78例重症急性胰腺炎患者,根据治疗方式的不同将其分为观察组(39例,早期腹腔微创引流联合连续性血液净化治疗)和对照组(39例,连续性血液净化治疗)。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0.05)。治疗48 h后,观察组的内皮素-1(ET-1)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平低于对照组,血管内皮生长因子(VEGF)、一氧化氮(NO)水平高于对照组(P<0.05)。观察组的治疗48 h后急性生理和慢性健康状况评分Ⅱ(APACHEⅡ)评分低于对照组,机械通气时间及重症监护病房(ICU)住院时间短于对照组(P<0.05);两组的28 d病死率比较,差异无统计学意义(P>0.05)。结论 早期腹腔微创引流联合连续性血液净化治疗重症急性胰腺炎患者可有效降低炎症因子水平,保护内皮细胞功能,促进机体恢复。
Objective To explore the effect of early minimally invasive abdominal drainage combined with continuous blood purification in the treatment of patients with severe acute pancreatitis.Methods A total of 78 patients with severe acute pancreatitis admitted in our hospital from October 2018 to October 2020 were divided into observation group(39 cases,early minimally invasive abdominal drainage combined with continuous blood purification treatment)and control group(39 cases,continuous blood purification treatment)according to different treatment methods.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After 48 h of treatment,the levels of endothelin-1(ET-1),interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the observation group were lower than those in the control group,and the levels of vascular endothelial growth factor(VEGF)and nitric oxide(NO)were higher than those in the control group(P<0.05).The Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)score of the observation group was lower than that of the control group after 48 h of treatment,the mechanical ventilation time and intensive care unit(ICU)hospitalization time were shorter than those of the control group(P<0.05);there was no significant difference in 28 d mortality between the two groups(P>0.05).Conclusion Early minimally invasive abdominal drainage combined with continuous blood purification in the treatment of patients with severe acute pancreatitis can effectively reduce the levels of inflammatory factors,protect the function of endothelial cells,and promote the recovery of the body.
