摘要
文章介绍张唐法教授运用针刺、推拿综合治疗儿童多发性抽动症的临证经验,对其学术观点、诊治思路、选穴特点、特色针刺手法进行归纳总结,并选取医案一则加以佐证。张唐法教授认为儿童多发性抽动症与儿童先天禀赋和后天情志失调、饮食不节所导致脾肾亏虚、髓脑不足,引动肝风,神窍受扰有关。在治疗上以“治神”为准则,辨证论治,病症结合,治疗此病。所举医案为脾虚肝亢患儿,治疗上以补脾平肝为原则,使肝风得熄,脾气得健,则抽动可止。张唐法教授在治疗儿童多发性抽动症注重头针、小儿推拿以及体针的综合论治,手法上运用特色快速提插捻转手法进行头针操作,并配合“健脾补肾”体针法和小儿推拿手法进行操作。形成了头针体针并举,调神养神共行来治疗儿童多发性抽动症的特色治疗手段。
This article introduced Professor Zhang Tangfa's clinical experience in the comprehensive treatment of multiple Tourette syndrome in children using acupuncture and Tuina and summarized his academic viewpoints,diagnostic and treatment approaches,selection of acupoints,and unique needling manipulations,supported by a selected medical case.Professor Zhang Tangfa believes that multiple Tourette syndrome in children is related to spleen and kidney deficiency and insufficient marrow and brain caused by congenital constitution,postnatal emotional imbalance,and irregular diet,leading to the liver wind and spirit disturbance.In the treatment,the guiding principle is to“treat the spirit”,apply syndrome differentiation,combine syndrome and disease,and address the disease accordingly.The presented medical case study involves a child with spleen deficiency and liver hyperactivity.The treatment principle focuses on tonifying the spleen and calming the liver,thus extinguishing liver wind and strengthening the spleen qi.Professor Zhang Tangfa emphasizes a comprehensive approach using scalp acupuncture,pediatric Tuina,and body acupuncture in the treatment of multiple Tourette syndrome in children.He employs unique rapid lifting,thrusting,and twirling methods for scalp acupuncture,combined with the“spleen and kidney-tonifying”body acupuncture method and pediatric Tuina techniques.This combination of scalp and body acupuncture,along with the nourishment of the spirit,forms the distinctive therapeutic approach for treating multiple Tourette syndrome in children.
