摘要
目的:对比分析磁共振常规视野扩散加权成像(f-FOV DWI)与小视野扩散加权成像(r-FOV DWI)在宫颈癌诊断中的应用价值。方法:回顾性搜集本院2021年3月至2022年2月明确诊断为宫颈癌的患者,选取病理活检、磁共振检查均为宫颈癌阳性的患者140例,所有患者在进行手术治疗之前均进行f-FOV DWI和r-FOV DWI检查,对检查结果进行主观及客观评价。主观评价由2位影像科医师共同阅片,然后从解剖细节显示、磁敏感伪影、几何变形和总体诊断信心等四方面进行评分,分数越高代表成像效果越好。客观评价由2位影像科医师对2种序列b值=800 s/mm^(2)的相关图像进行所需值测量,比较两者之间的信噪比(SNR)、对比噪声比(CNR)差异;测量ADC值,对比两种序列的ADC值在不同病理分级、临床分期中的差异。结果:主观评分结果显示,r-FOV DWI序列在解剖细节显示、磁敏感伪影、几何变形和总体诊断信心等四项评分(分别为2.86±0.82、2.76±0.77、2.98±0.78、2.87±0.88)均明显高于f-FOV DWI序列(分别为2.06±0.76、1.98±0.59、2.09±0.67、2.21±0.79),差异均有统计学意义(P均<0.05)。在客观评价方面,f-FOV DWI与r-FOV DWI的SNR值(分别为25.46±11.01、26.58±10.89)、CNR值(20.21±11.36、20.03±7.31)差异均无统计学意义(P>0.05);r-FOV DWI与f-FOV DWI在宫颈癌不同病理分型、临床分期之间的ADC值除Ⅲ期外差异均有统计学意义(P<0.05),r-FOV DWI测得的ADC值较f-FOV DWI更低。结论:r-FOV DWI较f-FOV DWI在宫颈癌的诊断过程中更具优势,图像更清晰,可以更好地辅助临床诊断。
Objective:To compare the value of full field of view diffusion weighted imaging(f-FOV DWI)with reduced field of view diffusion weighted imaging(r-FOV DWI)in diagnosis of cervical cancer.Methods:A retrospective analysis was conducted in 140 patients diagnosed with cervical cancer by pathology and magnetic resonance imaging(MRI)from March 2021 to February 2022.All patients underwent both f-FOV DWI and r-FOV DWI examinations before surgical treatment,and the results were subjectively and objectively evaluated.Subjective evaluation performed by two radiologists included four aspects:anatomical detail display,magnetic sensitivity artifacts,geometric deformation,and overall diagnostic confidence,with higher scores indicating better imaging quality.Objective evaluation was conducted by measuring the required values of two sequences with a b-value of 800s/mm^(2),comparing the signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)in two sequences,and measuring the apparent diffusion coefficient(ADC)values to compare the differences in two sequences in different pathological grades and clinical stages.Results:Subjective evaluation scores showed that there were significantly higher scores of the r-FOV DWI sequence(2.86±0.82,2.76±0.77,2.98±0.78,2.87±0.88)than that of f-FOV DWI sequence(2.06±0.76,1.98±0.59,2.09±0.67,2.21±0.79)in terms of anatomical detail display,magnetic sensitivity artifacts,geometric deformation with statistical significance(P<0.05).In terms of objective evaluation,there were no significant differences in SNR(25.46±11.01,26.58±10.89)and CNR(20.21±11.36,20.03±7.31)between f-FOV DWI and r-FOV DWI(P>0.05).However,the ADC values of r-FOV DWI were significantly lower than that of f-FOV DWI in different pathological grades and clinical stages of cervical cancer(P<0.05),except for stageⅢ.Conclusion:There are advantages for r-FOV DWI in providing clearer images and better assistance in clinical diagnosis than f-FOV DWI in diagnosis of cervical cancer.
