小视野弥散加权成像在乳腺癌诊断中的临床应用 被引量:10

The clinical application of small-field diffusion-weighted imaging in the diagnosis of breast cancer

摘要 目的探讨小视野弥散加权成像在乳腺癌诊断中的临床应用价值。方法收集2017年1月~2018年12月确诊为乳腺癌患者的196例为观察对象,均实施单次激发平面回波扩散加权成像(SS-EPI DWI)和小视野弥散加权成像(rFOV DWI)检查;对两种检查方法的图像质量情况以及ADC值进行比较分析。结果rFOV DWI检查乳腺癌腺体病灶对比度评分(3.56±0.48),解剖结构(3.44±0.61),伪影(3.63±0.72),图像质量评分(10.67±0.95);与Ss-EPI DWI检查对比度评分(3.09±0.55),解剖结构(3.03±0.63),伪影(3.02±0.41),图像质量评分(9.14±0.97);两组比较(Z=-7.263、-6.115、-8.412、-7.803,P<0.001)有统计学意义。rFOV DWI检查乳腺癌腺体ADC值(1.182±0.24)×10-3 mm 2/s,与Ss-EPI DWI检查乳腺癌腺体ADC值(1.232±0.19)×10-3 mm 2/s,比较差异有统计学意义(t=10.902,P<0.005)。rFOV DWI检查正常腺体ADC值(1.871±0.25)×10-3 mm 2/s,与Ss-EPI DWI检查正常腺体ADC值(1.945±0.29)×10-3 mm 2/s,比较差异无统计学意义(t=2.893,P>0.005)。结论乳腺癌实施小视野弥散加权成像检查对病灶的解剖结构、对比度等图像质量更优,对于微小病灶的检出率更高,临床检查中更具有优势。 Objective To explore the clinical value of small-field diffusion-weighted imaging in the diagnosis of breast cancer.Methods 196 patients with breast cancer diagnosed between January 2017 and December 2018 were enrolled,and single-shot plane echo diffusion-weighted imaging(SS-EPI DWI)and small-field diffusion-weighted imaging(rFOV DWI)were performed.We compared and analyzed the image quality and ADC values of the two inspection methods.Results rFOV DWI was used to examine the contrast score of breast gland lesions(3.56±0.48),anatomical structure(3.44±0.61),artifact(3.63±0.72),image quality score(10.67±0.95);contrast with Ss-EPI DWI Score(3.09±0.55),anatomical structure(3.03±0.63),artifact(3.02±0.41),and image quality score(9.14±0.97);comparison between the two groups(Z=-7.263,-6.115,-8.412,-7.803,P<0.001)was statistically significant.rFOV DWI was used to examine the breast cancer gland ADC value(1.182±0.24)×10-3 mm 2/s,and the breast cancer gland ADC value(1.232±0.19)×10-3 mm 2/s was compared with Ss-EPI DWI.The difference was statistically significant.Meaning(t=10.902,P<0.005).rFOV DWI was performed to check the normal gland ADC value(1.871±0.25)×10-3 mm 2/s,and the normal gland ADC value(1.945±0.29)×10-3 mm 2/s was compared with Ss-EPI DWI.The difference was not statistically significant(t=2.893,P>0.005).Conclusion The small-field diffusion-weighted imaging examination of breast cancer has better image quality,such as anatomical structure and contrast,and the detection rate of microscopic lesions is higher,which is more advantageous in clinical examination.
