摘要
目的观察消疣汤联合CO_(2)点阵激光治疗面部扁平疣的临床疗效及对皮损组织Toll样受体3(TLR3)、TLR9的影响。方法将120例面部扁平疣患者按照随机数字表法分为2组,对照组60例采用CO_(2)点阵激光治疗,治疗组60例在对照组治疗基础上予消疣汤治疗8周。比较2组治疗前及治疗4、8周后瘙痒、疼痛、皮损评分、皮损组织TLR3 mRNA、TLR9 mRNA、血清细胞因子[干扰素γ(IFN-γ)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)],并统计2组临床疗效、不良反应及复发率。结果治疗组总有效率83.33%(50/60),对照组总有效率66.67%(40/60),治疗组临床疗效优于对照组(P<0.05)。2组治疗4、8周后瘙痒、疼痛、皮损评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组同期(P<0.05)。2组治疗4、8周后皮损组织TLR3 mRNA、TLR9 mRNA表达均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组同期(P<0.05)。2组治疗4、8周后血清IL-6、TNF-α水平均较本组治疗前降低(P<0.05),IFN-γ水平均升高(P<0.05),且治疗组治疗后IL-6、TNF-α水平均低于对照组同期(P<0.05),IFN-γ水平高于对照组同期(P<0.05)。2组不良反应发生率及复发率比较差异无统计学意义(P>0.05)。结论消疣汤联合CO_(2)点阵激光有利于缓解面部扁平疣临床症状,提高治疗效果,机制可能与下调皮损组织TLR3、TLR9 mRNA有关。
Objective To observe the mechanism of Xiaoyou Decoction combined with CO_(2) lattice laser for facial flat wart and its effect on Toll-like receptor 3(TLR3)and Toll-like receptor 9(TLR9)in skin lesions.Methods A total of 120 patients with facial flat wart were randomly assigned into the treatment group(n=60)and the control group(n=60).All patients were managed by CO_(2) lattice laser,and those in the treatment group were additionally treated with Xiaoyou Decoction.Treated for 8 weeks,the aim was to compare the scores of itch,pain,skin lesions,TLR3,TLR9,serum cytokines(interferon-γ[IFN-γ]interleukin 6[IL-6],tumor necrosis factor alpha[TNF-α])on week 0 and on weeks 4 and 8.The curative effect,adverse reactions,recurrence rate were assessed.Results The overall effective rate in the treatment group was better than that in the control group(83.33%[50/60]vs 66.67%[40/60],[P<0.05],respectively).The scores of itch,pain,skin lesions on weeks 4 and 8 of treatment in the both groups were significantly decreased(P<0.05),the decrease was more pronounced in the treatment group than in the control group(P<0.05).The expression of TLR3 and TLR9 mRNA on weeks 4 and 8 in the both groups were significantly decreased(P<0.05),the treatment group was more prevalent(P<0.05).Significant decreased IL-6 and TNF-αand significant increased IFN-γon weeks 4 and 8 were detected in the both groups(all P<0.05),and the different was significant between groups for the above indexes(all P<0.05).The difference was not statistically significant in the incidence of adverse reactions and recurrence rate between groups(P>0.05).Conclusion For patients with facial flat wart,Xiaoyou Decoction combined with CO_(2) lattice laser is beneficial to relieve clinical symptoms,which can improve the therapeutic effect by downregulating the expression of TLR3 and TLR9.
