摘要
目的 探讨原发性肝癌(PLC)在碳离子治疗(CIT)前后的磁共振成像(MRI)中的表现特征及规律。方法 回顾性分析2018年12月至2022年4月在甘肃省武威肿瘤医院行住院治疗的26例单结节型PLC患者经CIT治疗前后的MRI检查资料,对比治疗前后数次的肿瘤轴位面积、体积、表观扩散系数(ADC)值,分析MRI影像学动态变化特点,并运用受试者工作特征(ROC)曲线计算曲线下面积、特异度和敏感度,得出ADC值预测CIT治疗PLC好转的最佳阈值。结果 PLC在CIT放疗后,与治疗前的基线期相比,肿瘤面积、体积不断缩小;而病灶ADC值逐渐上升,差异有统计意义(P<0.05);1.5~2个月、4.5~6个月复查时ADC值预测病变好转的阈值分别为0.806,0.739,敏感度、特异度分别69.60%、93.30%与78.30%、66.70%。结论 3.0T MRI功能成像在评估CIT治疗PLC的近中期疗效方面,为临床提供了客观、简捷的影像学依据,有很好的临床应用价值。
Objective To explore the characteristics and regularity of primary liver cancer(PLC)in magnetic resonance imaging(MRI)before and after carbon ion therapy(CIT).Methods The MRI data of 26 patients with single nodular PLC who were hospitalized in Gansu Wuwei Tumor Hospital from December 2018 to April 2022 before and after CIT were retrospectively analyzed.The axial area,volume and apparent diffusion coefficient(ADC)of the tumor before and after treatment were compared,and the dynamic changes of MRI imaging were analyzed.The receiver operating characteristic curve(ROC)was adopted to calculate the area under the curve,specificity and sensitivity,and the optimal threshold of ADC value to predict the improvement of PLC treated by CIT was obtained.Results Compared with the baseline period before treatment,the tumor area and volume of PLC decreased after CIT radiotherapy,while the ADC value of the lesion gradually increased,with statistically significant differences.At 1.5-2 months and 4.5-6 months after treatment,the thresholds of ADC value for predicting the improvement of lesion were 0.806 and 0.739 respectively,the sensitivity were 69.60%and 93.30%respectively,and the specificity were 78.30%and 66.70%respectively.Conclusion 3.0T MRI functional imaging provides an objective and simple imaging basis for clinical application in evaluating the short-term and medium-term efficacy of CIT therapy for PLC,and has good clinical application value.
