幽门螺杆菌感染与脑梗死的关系及其对血管内皮功能、氧化应激状态的影响 被引量:5

The relationship between Helicobacter pylori infection and cerebral infarction and its influence on vascular endothelial function and oxidative stress status

摘要 目的 探讨幽门螺杆菌(Hp)感染与急性脑梗死(ACI)的关系及其对血管内皮功能、氧化应激状态的影响。选取2021年3月至2022年6月合肥市第八人民医院收治的92例初发性ACI患者(脑梗死组),同期92例健康体检者(对照组)。比较两组Hp感染情况,根据病情严重程度将脑梗死组分为未感染组(n=40)、轻度感染组(n=31)和重度感染组(n=21),比较各组血管内皮功能、氧化应激状态及血清炎性因子水平。脑梗死组Hp感染52例,其中重度感染21例,HpⅠ型感染33例;对照组Hp感染31例,重度感染9例,HpⅠ型感染20例,两组重度感染率及Hp感染率比较差异有统计学意义(P<0.05)。小动脉闭塞型组Hp感染率低于大动脉粥样硬化型组(P<0.05),两组感染严重程度比较差异无统计学意义(P>0.05)。脑梗死组不同Hp感染严重程度患者的一氧化氮、血管内皮生长因子、超氧化物歧化酶、谷胱甘肽过氧化物酶水平比较,重度组<轻度组<未感染组(P<0.05);内皮素-1、丙二醛、肿瘤坏死因子-α、超敏C反应蛋白、同型半胱氨酸水平比较,重度组>轻度组>未感染组(P<0.05)。Hp感染可增强ACI患者血管内皮损伤及氧化应激状态,提高血清炎性因子水平,促进疾病进展。 Objective To explore the relationship between Helicobacter pylori(Hp)infection and acute cerebral infarction(ACI)and the influence of Hp infection on vascular endothelial function and oxidative stress status.Methods Ninety-two patients with initial ACI admitted to the Hefei Eighth People’s Hospital from March 2021 to June 2022 were selected as a cerebral infarction group,and 92 healthy subjects with physical examination were selected as a control group.Hp infection was compared between the two groups.According to the severity of disease,the patients in the cerebral infarction group were further divided into an uninfected group(n=40),a mildly infected group(n=31)and a severely infected group(n=21).The vascular endothelial function,oxidative stress status and serum inflammatory factors were compared among the various groups.Results There were 52 patients with Hp infection in the cerebral infarction group,including 21 with severe infection and 33 with Hp type I infection.There were 31 patients with Hp infection in the control group,including 9 with severe infection and 20 with Hp type I infection.There were statistical differences in the severe infection rate and Hp infection rate between the two groups(P<0.05).Small-artery occlusion group had a significantly lower Hp infection rate than large-artery atherosclerosis group did(P<0.05).However,there was no statistical difference in the severity of infection between the two groups(P>0.05).Comparison of levels of nitric oxide(NO),vascular endothelial growth factor(VEGF),superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)in patients with different Hp infection severity in the cerebral infarction group showed that the severe group<the mild group<the uninfected group(P<0.05).Comparison of levels of endothelin-1(ET-1),malondialdehyde(MDA),tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP)and homocysteine(Hcy)showed that the severe group>the mild group>the uninfected group(P<0.05).Conclusions Hp infection can enhance the vascular endothelial injury and oxidative stress status in patients with ACI.However,It increases the levels of serum inflammatory factors and promotes the progression of disease.
