摘要
肠道微生物在机体营养与代谢中起到重要作用,且现代研究表明肠道微生物和中医理论中的脾胃概念有相似之处。肠道微生物位于肠中,但其功能属脾胃,中医药可通过特定通路及作用机制调节肠道菌群,从而达到改善强直性脊柱炎(ankylosing spondylitis,AS)症状的目的。临证多从益肾壮督理论辨证论治AS,以补肾祛寒、强督助阳为治疗大法,同时辅以化湿疏风、荣筋通络之法,治疗过程中强调注重调护脾胃以固后天之本。中医药基于脾胃理论干预肠道菌群治疗AS效果肯定,但目前研究仍不完善,如缺乏大样本数据;各项研究结果差异较大;部分菌群益害种类划分尚不完全明确等。肠道菌群调节AS患者机体免疫系统的研究仍需进一步探讨,以期为AS的预防、治疗、预后等提供更有力的证据和突破点。
Intestinal microorganisms play an important role in body nutrition and metabolism,and modern research shows that intestinal microorganisms are similar to the concept of Spleen and Stomach in TCM theory.Intestinal microorganisms are located in the intestine,but their functions belong to the Spleen and Stomach.Traditional Chinese medicine can regulate the intestinal flora through specific pathways and mechanisms so as to improve the symptoms of ankylosing spondylitis(AS).Clinical treatment of AS is based on the theory of tonifying Kidney and strengthening Du meridian,with tonifying Kidney and dispelling Cold,strengthening Du meridian and helping Yang as the treatment method,supplemented by the methods of eliminating Dampness and expelling Wind,and nourishing tendons and dredging collaterals.In the treatment process,emphasis is placed on nursing Spleen and Stomach to consolidate the foundation of acquired constitution.Based on the theory of Spleen and Stomach,the intervention of traditional Chinese medicine on intestinal flora in the treatment of AS is effective,but the current research is still not perfect,such as the lack of large sample data,the research results are quite different,the classification of beneficial and harmful species of some bacteria groups is not completely clear.The study of intestinal flora regulating the immune system of AS patients still needs further discussion in order to provide more powerful evidence and breakthrough points for the prevention,treatment and prognosis of AS.
