摘要
目的探讨经颅彩色多普勒(transcranial color-coded duplex sonography,TCCD)在慢性肾脏病(chronic kidney disease,CKD)患者脑动脉血流动力学评估中的应用价值。方法回顾性分析2022年5月—2023年3月牡丹江医学院附属红旗医院双侧颈动脉及颅内大血管均未狭窄及闭塞的126例脑血管病变的患者,并且根据是否伴有慢性肾病分为CKD组73例和非CKD组53例,应用TCCD和颈部血管超声对所有患者进行检查,测量两组人员双侧颈动脉和颅内大血管频谱数据并得出血管RI及PI,将所得数据进行统计学分析。结果CKD组患者左侧和右侧前动脉PI值均低于非CKD组,差异有统计学意义(P<0.05)。左侧大脑中动脉PI值(1.03±0.176)及右侧大脑中动脉PI值(1.00±0.173)高于非CKD组(0.90±0.15)和(0.92±0.17),差异有统计学意义(t=4.350、2.581,P<0.05)。结论TCCD能够对CKD患者的脑血管血流动力学改变进行评估,从而为临床下一步诊疗方案提供参考依据。
Objective To investigate the application value of transcranial color-coded duplex sonography(TCCD)in the assessment of cerebral arterial hemodynamics in patients with chronic kidney disease(CKD).Methods A retrospective analysis was performed on one hundred and twenty-six patients with cerebrovascular diseases in Hongqi Hospital Affiliated to Mudanjiang Medical College from May 2022 to March 2023 who had no stenosis or occlusion of bilateral carotid arteries and intracranial large vessels,and were divided into seventy-three patients in the CKD group and fifty-three patients in the non-CKD group according to whether they were accompanied by chronic kidney disease.All patients were examined by TCCD and cervical vascular ultrasound.The spectrum data of bilateral carotid arteries and intracranial great vessels of the two groups were measured,and vascular RI and PI were obtained,and the obtained data were statistically analyzed.Results The PI values of in both the left and right anterior cerebral artery in CKD patients were significantly lower than those in non-CKD group,and the difference was statistically significant(P<0.05).The left middle cerebral artery PI values(1.03±0.176)and right middle cerebral artery PI values(1.00±0.173)in CKD group were higher than those in non-CKD group(0.90±0.15)and(0.92±0.17),the difference was statistically significant(t=4.350,2.581,P<0.05).Conclusion TCCD can evaluate the changes of cerebrovascular hemodynamics in patients with CKD,so as to provide reference for the next clinical diagnosis and treatment plan.
