摘要
提出治疗儿童多发性抽动症应予以“脏腑平治”。认为本病与五脏失常相关:肺失宣肃,气不下达而上扰可见不自主动作;心火旺盛,心神不能归脏腑可见情志异常;肝阴不足或肝火亢盛,阴不能潜阳,筋脉失养可见抽动频发;脾失健运生痰、脾虚无力使气机升降失常以致神明昏蒙而见秽语、专注力异常;肾虚伴肝阴虚,先天失养,精髓不生,一身精神活动失根而虚浮,则精神、动作失常。临证具体证候亦有多样化表现,如注意力不集中与脾藏意、肾藏志相关,不自主动作与肺魄、肝魂妄动相关,故以“脏腑平治”之法,从心、从肺、从肝、从脾、从肾论治,并重视五脏之间的相互关联,随证治之。
It is proposed that“even treatment of zang-fu organs((脏腑)”should be taken in management of chil-dren with Tourette disease.Abnormality of the five zang(月)organs is associated with this disease.When the lungs fail to diffuse and purify,and qi cannot flow down and harass the upper body,involuntary movements will then be seen.Vigorous heart fire and heart spirit failing to enter the zang-fu organs will cause abnormal emotions.Liver yin in-sufficiency or hyperactivity of liver fire will cause yin failing to subdue yang and the loss of the sinews nourishment,resulting in frequent twitches.When the spleen fails to transport,and phlegm is induced,ascending and descending of qi movement will be disordered due to spleen deficiency and weakness,which will cause mental confusion,obscene language and abnormal concentration.If there is kidney deficiency and liver yin deficiency,loss of congenital nourish-ment,no production of essence and marrow,and loss of the roots in mental activities will result in mental and move-ment disorders.Clinically,there are diversity performance in the manifestations of specific syndromes,such as inat-tention in relation to spleen storing reflection and kidney storing will,and involuntary movement in relation to frenetic stirring of lung corporeal soul and liver ethereal soul.Therefore,“even treatment of zang fu organs"is suggested,treating disease according to the syndromes from heart,lung,liver,spleen,kidney,and importance should be attached to the association of five zang.
