摘要
脑卒中后认知障碍是由于患者脑组织缺血而导致脑细胞坏死,出现认知功能障碍的一种急性脑血管疾病,可能导致患者残疾,降低患者的生活质量、缩短患者的存活时间,给家庭及社会带来沉重负担。此病近年来受到国内外极大的关注。现代医学目前尚未发现治疗本病的特效药物。中医学并没有专门记载脑卒中后认知障碍的文献,通过其在临床上的记忆力降低为主的表现,可将其归属于“健忘”“痴呆”等范畴。文章认为其发生的根本因素是心脾肾亏虚,痰浊、瘀血又是其脏腑功能失调的病理产物,因虚致实,虚实错杂,为本虚标实之证。张捷教授从事脑病临床工作30余年,结合中医经典理论与多年临床经验,提出了脏腑-督脉-脑一体论的理论,将督脉作为脏腑与脑之间的调控枢纽,认为督脉是沟通脏腑与脑功能活动的桥梁。起病时,疏通痹阻之督脉以修复脑脉,在临床中运用针药结合的方法治疗卒中后认知障碍,取得较好的疗效。文章通过案例介绍张捷教授治疗卒中后认知障碍的经验,以期为临床提供借鉴。
Post-stroke cognitive impairment is an acute cerebrovascular disease caused by brain cell necrosis after ischemia of brain tissue,with cognitive dysfunction as the main symptoms.The disease may lead to disability of patients,reduce quality of life of the patient,shorten the survival time,and bring heavy burdens to society and families.In recent years,the disease has received great attention at home and abroad.Modern medicine has not yet found a specific medicine to treat the disease.TCM does not specifically record the literature on cognitive impairment after stroke,and it can be classified as forgetfulness and dementia through its clinical memory loss.This paper believes that the fundamental factors of its occurrence are the deficiency of the heart,spleen and kidney,and the turbidity of sputum and blood stasis are the pathological products of the dysfunction of its internal organs.Professor ZHANG Jie has been engaged in the clinical work of encephalopathy for more than 30 years,combining the classical theory of TCM and many years of clinical experience,the theory of the integration of internal organs-supervision and brain is proposed,which takes the supervision pulse as the regulatory hub between the internal organs and the brain,and believes that the supervision pulse is a bridge to communicate the functional activities of the internal organs and the brain.At the onset of the disease,unblock the obstructed veins to repair the cerebral veins,and has gained good curative effects in the clinical treatment of post-stroke cognitive impairment by the combination of acupuncture and medicine.In this paper,Professor ZHANG Jie’s clinical experience in the treatment of post-stroke cognitive impairment is introduced through an empirical case study,in order to provide a clinical reference.
