摘要
目的:探析高频彩超及超声造影(CEUS)对不完全睾丸扭转(TT)的诊断效能。方法:选择医院收治的80例不完全TT患者,所有患者均行高频彩超及CEUS扫描,对比患者健侧及患侧高频彩超睾丸检测结果及睾丸实质CEUS定量参数,分析不完全TT患者高频彩超及CEUS影像特征,并对比两种超声技术的诊断效能。结果:不完全TT患者患侧与健侧高频彩超及CEUS血流信号等级差异有统计学意义(Z=110.306,P<0.05),患侧血流信号主要集中于Ⅰ级,而健侧血流信号主要集中于Ⅲ级。两侧到达时间(AT)及达峰时间(TTP)定量参数比较差异无统计学意义;而患侧流出时间(WT)明显长于健侧,峰值强度(PI)明显低于健侧,差异有统计学意义(t=8.26,t=6.34;P<0.05)。CEUS与高频彩超诊断特异差异无统计学意义(P>0.05),CEUS的准确率及灵敏度均明显高于高频彩超,差异有统计学意义(x^(2)=7.828,x^(2)=6.871;P<0.05)。结论:CEUS应用于不完全TT中诊断效能明显优于高频彩超,诊断准确度及灵敏度更高,值得临床广泛推广。
Objective:To explore the diagnostic efficiency of high-frequency color Doppler ultrasound and contrastenhanced ultrasound(CEUS)in incomplete testicular torsion(TT).Methods:A total of eighty patients with incomplete TT who admitted to hospital were selected as the research objects.All patients underwent high-frequency color Doppler ultrasound and CEUS scan.The test results of high-frequency color Doppler ultrasound and the quantitative parameters of testicular parenchymal ultrasound were compared between the health side and affected side of patients.The characteristics of high-frequency color Doppler ultrasound and CEUS of patients with incomplete TT were analyzed,and the diagnostic efficiencies of the two kinds of ultrasound techniques were compared.Results:The difference of blood flow signal level between the health side and the affected side of patients with incomplete TT was significant(Z=110.306,P<0.05).The blood flow signal of the affected side of them mainly concentrated in grade I,while the blood flow signal of the health side of them mainly concentrated in grade III.The differences of arrival time(AT)and time to peak(TTP)quantitative parameter between the two sides were no significant(P>0.05),while the outflow time(WT)of the affected side was significantly longer than that of the healthy side,and the peak intensity(PI)of the former was significantly lower than that of the healthy side(t=8.26,t=6.34,P<0.05).There were no statistical differences in the diagnostic specificity between CEUS and high-frequency color Doppler ultrasound(P>0.05),while the accuracy and sensitivity of CEUS were significantly higher than those of high-frequency color Doppler ultrasound(x^(2)=7.828,x^(2)=6.871,P<0.05),respectively.Conclusion:The diagnostic efficiency of CEUS that is used in incomplete TT is significantly better than that of high-frequency color Doppler ultrasound,which diagnostic accuracy and sensitivity are higher,and it is worth to be widely promoted in clinical work.
