摘要
目的观察多模态影像[氟代脱氧葡萄糖(18F-FDG)PET/CT联合增强CT]指导下^(125)I粒子植入治疗肺部恶性实体肿瘤的临床短期疗效。方法肺部恶性实体肿瘤患者54例(共60个病灶)按照^(125)I粒子植入前靶区勾画方式不同分为两组,A组26例(30个病灶)在^(18)F-FDG PET/CT联合增强CT指导下勾画生物靶区和临床靶区,B组28例(30个病灶)仅在增强CT指导下勾画临床靶区。两组均行^(125)I粒子植入治疗,比较两组植入粒子数和每立方厘米植入粒子数、穿刺并发症及术后第1、2、6个月临床短期疗效。结果两组均顺利完成^(125)I粒子植入。A组植入粒子数和每立方厘米植入粒子数均少于B组[(67.63±36.53)颗vs.(94.83±40.71)颗和(0.84±0.08)颗vs.(1.10±0.08)颗](P<0.05)。A组穿刺并发症发生率与B组相仿(15.38%vs.25.00%)(P>0.05)。A组术后第6个月病灶疾病控制率高于B组(93.33%vs.73.33%)(P<0.05)。结论多模态影像指导下^(125)I粒子植入治疗肺部恶性实体肿瘤的临床短期疗效优于单纯增强CT,可有效减少^(125)I粒子植入数量。
Objective To observe clinical short-term efficacy of^(125)I seed implantation under multimodal image guidance of 18F-flurodeoxyglucose(18 F-FDG)PET/CT and enhanced CT in the treatment of lung malignant solid tumors.Methods According to different delineation methods of target areas before^(125)I seed implantation,54patients(60lesions)with lung malignant solid tumors were divided into two groups.Biological target areas and clinical target areas were delineated under a guidance of 18F-FDG PET/CT and enhanced CT in group A(26cases,30lesions).Clinical target areas were delineated under a guidance of enhanced CT alone in group B(28cases,30lesions).^(125)I seed implantation was performed in both groups.The number of implanted seeds,the number of implanted seeds per cubic centimeter,complications of puncture and short-term clinical efficacy in the 1st,2nd and 6th month after surgery were compared between the two groups.Results 125 I seed implantation was successfully completed in both groups.The number of implanted particles and the number of implanted particles per cubic centimeter in group A were less than those in group B[(67.63±36.53)particles vs.(94.83±40.71)particles and(0.84±0.08)particles vs.(1.10±0.08)particles](P<0.05).The incidence of puncture complications in group A was similar to that in group B(15.38%vs.25.00%)(P>0.05).Disease control rate of the lesions in group A was higher than that in group B in the 6th month after surgery(93.33%vs.73.33%)(P<0.05).Conclusion The short-term clinical efficacy of^(125)I seed implantation under multimodal image guidance in the treatment of lung malignant solid tumors is better than that of enhanced CT alone,which can effectively reduce the number of implanted^(125)I seed.
