摘要
介绍从“风毒伏络”论治三叉神经痛的临证体会。认为外受风毒之邪为三叉神经痛发生的主要原因,风毒伏络为本病核心病机。头面为诸阳之汇、络脉网布,位于高巅,易受风毒。风毒入络,阻滞经气,不通则痛;风毒伤卫,邪气不得清除,伏于络中,故多缠绵难愈。临床进行分期论治,发作期止痛为要,以风药祛风、血药活络、虫药攻毒,共奏祛风攻毒活络之功以缓急止痛;缓解期调本为主,固表养络、调和营卫以防邪复。
This paper summarized the treatment experience of trigeminal neuralgia on the basis of“latent wind-toxin in collaterals”.It is considered that external invasion of pathogenic wind-toxin is the main cause of trigeminal neuralgia,and latent wind-toxin in collaterals is the core pathogenesis.The head and face are the confluence of all yang and the network of collaterals,which is located on the top and is vulnerable to wind-toxin.Wind-toxin invades the collaterals and blocks channel qi,and if the flow of channel qi is not smooth,the pain occurs.Since the defensive qi is damaged by wind-toxin,the pathogen can not be removed,lying in the collaterals and being difficult to heal.Treatment by stages is suggested.Pain relief during the attack period is important.In order to relax the tension and relieve pain,wind medicals,blood medicinals and insect medicinals should be used together by playing their role each in dispelling wind,activating collaterals,and attacking toxin and achieving the integrated effect.In remission period,it is mainly to adjust the constitution.To prevent recurrence,the method of consolidating the exterior and nourishing collaterals as well as harmonize ying(营)and wei(卫)levels should be used.
