摘要
目的 探究乳腺癌术后上肢淋巴水肿患者应用泡沫块绷带联合高弹力绷带包扎方法进行包扎处理后,对患者的临床应用效果及对患者皮肤质地的影响。方法 选取2021-01-01-2022-05-30福建省三明市沙县区总医院103例乳腺癌术后上肢淋巴水肿患者作为研究对象,根据随机数字分组法将其分为对照组和观察组,其中对照组51例给予中低密度平层敷料为衬垫联合弹力绷带包扎,观察组52例给予泡沫块绷带与高弹力绷带联合包扎。应用单因素协方差对比2组患者淋巴水肿程度、皮肤质地、紧绷感、麻木感、肿胀感、并发症发生情况。结果 实施干预后,观察组患者手掌周径(16.71±0.88) cm,低于对照组的(17.18±0.49) cm,差异有统计学意义,F=3.346,P<0.001;肘部周径(20.14±3.37) cm低于对照组的(22.26±3.41) cm,差异有统计学意义,F=3.167,P<0.001;肘上10 cm周径(30.08±1.24) cm,低于对照组的(32.19±1.31) cm,差异有统计学意义,t=8.376,P<0.001。干预后,观察组患者皮肤水分含量(48.97±2.28)%,低于对照组(50.08±2.36)%,差异有统计学意义,F=2.419,P<0.001;经皮水分蒸发速率(10.03±2.31) g·m^(-2)·h^(-1),低于对照组(11.29±2.48) g·m^(-2)·h^(-1),差异有统计学意义,F=2.657,P<0.001;皮肤纤维化程度(0.05±0.01) N,低于对照组的(0.06±0.02) N,差异有统计学意义,F=3.187,P<0.001。干预后观察组上肢淋巴回流量为(15.72±2.24) mL/2 h,对照组为(11.57±1.36) mL/2 h,差异有统计学意义,F=1.389,P<0.001。2组患者住院期间并发症的总发生率对比,差异无统计学意义,χ^(2)=0.287,P>0.05。结论 应用泡沫块绷带联合高弹力绷带包扎方法对乳腺癌术后上肢淋巴水肿患者实施干预,可有效减轻淋巴水肿程度,改善皮肤质地,且干预安全性较高。
Objective To explore the clinical effect and influence on skin texture of patients with upper limb lymphedema after breast cancer surgery by applying foam block bandage combined with high elastic bandage.Methods A total of 103patients with upper limb lymphedema after breast cancer surgery from 2021-01-01to 2022-05-30 in Shaxian District General Hospital,Sanming City,Fujian Province,were selected as the study objects.According to random number grouping,they were divided into control group and observation group.The control group was treated with low density flat dressing as pad combined with elastic bandage.The observation group was treated with a combination of foam bandage and high elastic bandage.Univariate covariance was used to compare the degree of lymphedema,skin texture,tightness,numbness,swelling,and lymphedema related symptoms between the two groups.Results After the intervention,the palm circumference in the observation group(16.71±0.88)cm was lower than that in the control group(17.18±0.49)cm,the difference was statistically significant,F=3.346,P<0.001;The circumference of the elbow (20.14±3.37)cm was lower than that of the control group(22.26±3.41)cm,and the difference was statistically significant,F=3.167,P0.001;The circumferential diameter of 10cm above the elbow (30.08±1.24)cm was lower than that of the control group(32.19±1.31)cm,the difference was statistically significant,t=8.376,P0.001.After the intervention,the skin moisture content of observation group(48.97±2.28)%was lower than that of control group(50.08±2.36)%,the difference was statistically significant,F=2.419,P<0.001;The transcutaneous water evaporation rate(10.03±2.31)g·m^(-2)·h^(-1) was lower than that of the control group(11.29±2.48)g·m^(-2)·h^(-1),and the difference was statistically significant,F=2.657,P<0.001;The degree of skin fibrosis(0.05±0.01)N was lower than that of the control group(0.06±0.02)N,and the difference was statistically significant,F=3.187,P<0.001.After intervention,the upper limb lymphatic reflux flow was(15.72±2.24)ml/2hin the observation group and(11.57±1.36)ml/2hin the control group.The difference was statistically significant(F=1.389,P<0.001).The total incidence of complications during hospitalization was compared between the two groups,the difference was statistically insignificant(χ^(2)=0.287,P>0.05).Conclusion The application of foam bandages combined with high elastic bandages for upper limb lymphedema patients after breast cancer surgery can effectively reduce the degree of lymphedema,improve the texture of the skin,and the intervention is safe.
