摘要
目的探讨血常规与C反应蛋白(C-reactive protein,CRP)联合诊断儿童上呼吸道感染的临床诊断价值。方法本次研究对象为徐州市矿山医院检验科2021年1月—2023年1月收治的83例上呼吸道感染患儿,依据疾病类型分组,其中40例为细菌感染,43例为病毒或支原体等非细菌感染;另选取同期于徐州市矿山医院体检的40名健康儿童设为对照组,检测与比较三组血常规与CRP水平。结果与健康组相比,细菌感染组与非细菌感染组中性粒细胞(neutrophil,NEU)与白细胞计数(white blood cell count,WBC)及CRP水平均更高(P<0.05),且细菌感染组上述指标水平高于非细菌感染组(P<0.001);血常规联合CRP诊断儿童上呼吸道感染的灵敏度、阳性与阴性预测值及诊断符合率均高于血常规单独检测(P<0.05),亦高于CRP单独检测,但比较差异无统计学意义(P>0.05)。结论血常规联合CRP诊断儿童上呼吸道感染的检测价值较高,可提高诊断符合率,便于指导临床合理用药。
Objective To investigate the clinical value of blood routine and C-reactive protein(CRP)in the diagnosis of upper respiratory tract infection in children.Methods The subjects of this study were 83 children with upper respiratory tract infection admitted to the department of clinical laboratory,Xuzhou Mining Hospital from January 2021 to January 2023.They were divided into groups according to disease types,including 40 children with bacterial infection and 43 children with non-bacterial infection such as viral or mycoplasma.In addition,40 healthy children who underwent physical examination in Xuzhou Mining Hospital during the same period were selected as the control group,and the blood routine and CRP levels of the three groups were detected and compared.Results Compared with healthy group,neutrophil(NEU),white blood cell count(WBC)and CRP levels were higher in bacterial infection group and non-bacterial infection group(P<0.05).The level of the above indexes in the bacterial infection group was higher than that in the nonbacterial infection group(P<0.001).The sensitivity,positive and negative predictive values and diagnostic coincidence rate of blood routine combined with CRP in the diagnosis of upper respiratory tract infection in children were higher than that of blood routine alone(P<0.05),and higher than that of CRP alone,but there was no statistical significance(P>0.05).Conclusion Blood routine combined with CRP is of high value in the diagnosis of upper respiratory tract infection in children,which can improve the diagnosis coincidence rate and facilitate clinical rational drug use.
