不同体位固定在脊柱转移瘤调强放射治疗中的应用效果观察 被引量:1

Observation on the Effect of Different Body Position Fixation in Intensity Modulated Radiotherapy for Spinal Metastasis

摘要 目的分析脊柱转移瘤予以不同体位固定方式调强放射治疗中的应用效果观察。方法选取福建省肿瘤医院2021年3月—2022年2月收治的102例脊柱转移瘤患者为研究对象,根据随机数字表法分为两组。对照组(51例)采用真空袋体位固定技术,观察组(51例)采用热塑体膜固定技术。比较两组患者治疗后的不同方向(左右、前后、头脚)摆放体位的误差;比较两组患者治疗后的临床效果、并发症、患者的满意程度。结果治疗后,观察组对于不同方向摆放体位(左右、前后、头脚)的误差值分别为(2.60±0.45)mm、(2.68±0.37)mm、(2.58±0.35)mm,均低于对照组的(6.81±0.74)mm、(7.91±0.77)mm、(6.79±0.72)mm,差异有统计学意义(P<0.05);治疗后,观察组的临床总有效例数为40例,对照组为28例,观察组临床总有效率(78.43%)高于对照组(54.90%),差异有统计学意义(P<0.05);治疗后,观察组发生放射性肠炎、呕吐恶心、放射性肺炎、放射性食管炎等并发症总发生率低于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者的满意程度评分高于对照组,差异有统计学意义(P<0.05)。结论脊柱转移瘤采用热塑体膜固定技术的临床效果显著,可降低其不同方向摆放体位的误差和并发症,增加患者的满意程度。 Objective To analyze the effect of intensity-modulated radiotherapy(IMRT)with different body position fixation for spinal metastasis.Methods A total of 102 patients with spinal metastasis admitted to Fujian Cancer Hospital from March 2021 to February 2022 were selected as the study subjects.They were divided into two groups according to the random number table method.The control group(51 cases)used vacuum bag body position fixation technology,and the observation group(51 cases)used thermoplastic body film fixation technology.Compare the positioning errors of the two groups of patients in different directions(left and right,front and back,head and foot)after treatment.The clinical effects,complications and satisfaction of patients in the two groups were compared.Results After treatment,the error values of the observation group in different directions(left and right,front and back,head and foot)were(2.60±0.45)mm,(2.68±0.37)mm,and(2.58±0.35)mm,respectively,lower than those of the control group(6.81±0.74)mm,(7.91±0.77)mm,and(6.79±0.72)mm,with statistically significant differences(P<0.05).After treatment,the total clinical effective cases were 40 cases(78.43%)in the observation group and 28 cases(54.90%)in the control group.The total clinical effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the total incidence of radiation enteritis,vomiting,nausea,radiation pneumonia,radiation esophagitis and other complications in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the satisfaction score of patients in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The clinical effect of using thermoplastic membrane fixation technology for spinal metastasis is significant,which can significantly reduce the error and complications of positioning in different directions,and enhance the patient's satisfaction.
