摘要
目的 研究青光眼术后泪液内白细胞介素-6(IL-6)mRNA、碱性成纤维细胞生长因子(bFGF)mRNA及肿瘤坏死因子-α(TNF-α)mRNA表达与感染性眼内炎发生的关系。方法 选取2018年1月-2020年9月在郑州市第七人民医院眼科行青光眼手术的患者80例为研究对象,根据术后是否发生感染性眼内炎分为感染组9例和未感染组71例,比较两组患者IL-6 mRNA、bFGF mRNA及TNF-α mRNA表达,受试者工作特征(ROC)曲线分析上述指标对青光眼术后感染性眼内炎发生的诊断效能。结果 9例感染患者共分离出病原菌12株,包括7株革兰阳性菌、4株革兰阴性菌、1株真菌;单因素分析显示,年龄、受教育程度、糖尿病、高血压、手术时间和无菌操作等因素与术后感染性眼内炎相关(P<0.05);感染组患者IL-6 mRNA、bFGF mRNA及TNF-α mRNA表达高于未感染组(P<0.05);ROC曲线显示,预测术后感染性眼内炎其中bFGF mRNA检测曲线下面积(AUC)最大,预测疾病敏感度为88.89%。结论 IL-6 mRNA、bFGF mRNA及TNF-α mRNA在青光眼术后感染性眼内膜炎患者中表达较高,检测上述指标有助于疾病的诊断。
OBJECTIVE To explore the association of expressions of interleukin-6(IL-6) mRNA, basic fibroblast growth factor(bFGF) mRNA and tumor necrosis factor-α(TNF-α) mRNA in tears of glaucoma surgery patients with infectious endophthalmitis. METHODS A total of 80 glaucoma patients who received surgical procedures in ophthalmology department of the 7th People′ s Hospital of Zhengzhou from Jan 2018 to Sep 2020 were recruited as the research subjects and were divided into the infection group with 9 cases and the no infection group with 71 cases according to the status of postoperative infectious endophthalmitis. The expression levels of IL-6 mRNA, bFGF mRNA and TNF-α mRNA were compared between the two groups of patients. The efficiencies of the above indexes in diagnosis of postoperative infectious endophthalmitis were analyzed by means of receiver operating characteristic(ROC) curve. RESULTS Totally 12 strains of pathogens were isolated from the 9 patients with infection, 7 of which were gram-positive bacteria, 4 were gram-negative bacteria, and 1 was fungus. Univariate analysis showed that age, education level, diabetes mellitus, hypertension, operation duration and aseptic operation were associated with the postoperative infectious endophthalmitis(P<0.05). The expression levels of IL-6 mRNA, bFGF mRNA and TNF-α mRNA of the infection group were significantly higher than those of the no infection group(P<0.05). ROC curve analysis indicated that the areas under curves(AUCs) of the bFGF mRNA was the largest in prediction of postoperative infectious endophthalmitis, with the sensitivity 88.89%. CONCLUSION The glaucoma surgery patients with postoperative infectious endophthalmitis show high expression levels of IL-6, bFGF and TNF-α mRNA, and the detection of the above indexes may facilitate the diagnosis.
