摘要
目的:调研上海市35所公立三级医院国际医疗门诊服务提供情况,分析其影响因素并提出合理的对策建议。方法:对2019年以及2021年的国际医疗门诊服务量、国际医疗部门机构设置情况、签约的商业保险公司数量、开设的国际医疗门诊服务科室数量、投入的卫生人力进行描述性分析,并构建平衡面板数据,以国际医疗门诊服务量为因变量、医疗机构规模情况为控制变量、其他因素作为自变量,使用Stata 14.0利用固定效应模型回归分析,探索影响上海市公立三级医院国际医疗门诊服务的影响因素。结果:影响上海市公立三级医院国际医疗门诊服务的主要因素为国际医疗门诊服务提供的科室数量(P<0.05)、与医院签订合作协议的商业保险公司数量(P<0.05)以及是否设立独立的国际医疗门诊区域(P<0.05)。结论:为进一步提高上海市公立医院国际医疗门诊服务量,医疗机构应根据自身学科特色开设更多的国际医疗门诊科室,鼓励有条件的医疗机构开设独立的国际医疗门诊区域,提高国际医疗服务的影响力和患者的就医体验,同时不断推进公立医院与商业健康保险机构的合作,提高门诊服务量的同时实现双向共赢。
Objective: To investigate the provision of international medical outpatient services in 35 Grade Ⅲ public hospitals in Shanghai and analyze its influencing factors and propose reasonable countermeasures. Methods: Descriptive analyses were conducted on the volume of international medical outpatient services, the institutional setting of international medical departments, the number of commercial insurance companies contracted,the number of international medical outpatient service departments opened, and the health technicians invested in 2019 and 2021, and balanced panel data were constructed, with the volume of international medical outpatient services as the dependent variable, the size of medical institutions as the control variable and other factors as the Stata 14.0 was used to explore the factors influencing international medical outpatient services in Grade III public hospitals in Shanghai, using fixed effects model regression analysis. Results: The main factors affecting international medical outpatient services in Grade Ⅲ public hospitals in Shanghai were the number of departments offering international medical outpatient services(P < 0.05), the number of commercial insurance companies with which the hospital had signed partnership agreements(P < 0.05) and whether a separate international medical outpatient area was established(P< 0.05). Conclusion: In order to further increase the volume of international medical outpatient services in public hospitals in Shanghai, medical institutions should open more international medical outpatient departments according to their own disciplinary characteristics, encourage qualified medical institutions to open independent international medical outpatient areas,improve the influence of international medical services and patients ’ medical experience, and continuously promote the cooperation between public hospitals and commercial health insurance institutions to increase the volume of outpatient services while achieving a two-way win-win situation.
