摘要
目的:调查湘西地区类风湿关节炎(RA)患者的诊疗现况及功能障碍、残疾情况,分析残疾的相关影响因素。方法:选取2021年2-8月在湘西自治州人民医院就诊的RA患者为研究对象,记录患者的一般情况、诊疗情况、功能受限、残疾情况,依据现行国家标准对患者残疾程度进行分级,并对残疾的相关影响因素进行分析。结果:共调查102例RA患者,年龄40~80岁,79.4%的患者首诊在风湿科以外的科室,41.2%的患者从未规律服用抗风湿药物;中重度功能障碍的比例为52.93%,存在残疾的比例为42.16%,一、二、三、四级残疾的比例分别为16.67%、40.48%、30.95%、11.90%。病程、报销方式是残疾的独立影响因素。结论:湘西地区RA患者诊疗不规范,功能受限及残疾发生率高,残疾程度重,地区需加强风湿科学科建设、加大宣传力度、提高医生及患者规范诊疗的意识,以降低RA残疾的发生风险。
Objective:To investigate the diagnosis,treatment,dysfunction and disability of patients with rheumatoid arthritis(RA)in western Hunan region,and to analyze the related influencing factors of disability.Methods:The patients with RA who were treated in People’s Hospital of Xiangxi Tujia and Miao Autonomous Prefecture from February 2021 to August 2021 were selected as the study subjects.The general condition,diagnosis and treatment,functional limitation and disability of patients were recorded.The degree of disability was classified according to the current national standards,and the related influencing factors of disability were analyzed.Results:A total of 102 patients with RA were investigated,they aged 40~80 years.79.4%of the patients were first diagnosed in departments other than rheumatology department,and 41.2%of the patients never used antirheumatic drugs regularly.The proportion of moderate and severe dysfunction was 52.93%,the proportion of disability was 42.16%,and the proportion of gradeⅠ,Ⅱ,ⅢandⅣdisability was 16.67%,40.48%,30.95%and 11.9%,respectively.The course of disease and the mode of reimbursement were independent influencing factors of disability.Conclusion:The diagnosis and treatment of RA patients in western Hunan region is not standard,the incidence of functional limitation and disability is high,and the degree of disability is serious.It is necessary to strengthen the discipline construction of rheumatology department,strengthen publicity and improve the awareness of standardized diagnosis and treatment of doctors and patients in order to reduce the risk of RA disability.
