3D打印多孔钛合金椎间融合器在颈椎前路椎间盘切除植骨融合治疗颈椎病术中的应用 被引量:9

Application of 3D print porous titanium alloy intervertebral fusion cage in anterior cervical discectomy and bone graft fusion for treatment of cervical spondylopathy

摘要 目的探讨颈椎前路椎间盘切除植骨融合术中应用3D打印多孔钛合金椎间融合器治疗颈椎病的临床疗效与安全性。方法纳入自2018-06—2020-06行颈椎前路椎间盘切除植骨融合术治疗的67例颈椎病,按随机数字表法分为试验组(33例)与对照组(34例),试验组术中置入3D打印多孔钛合金椎间融合器,对照组置入聚醚醚酮椎间融合器。比较两组手术时间、术中出血量、颈椎JOA评分、颈椎矢状位参数(融合节段高度和C_(2~7)Cobb角)、椎间融合情况。结果两组手术均顺利完成,切口均一期愈合,随访时间为(14.6±4.2)个月,期间均无明显并发症发生。两组手术时间、术中出血量、术后6个月颈椎JOA评分比较差异无统计学意义(P>0.05)。两组术后1周的融合节段高度、C_(2~7)Cobb角比较差异无统计学意义(P>0.05),试验组术后3个月及术后6个月的融合节段高度、C_(2~7)Cobb角高于对照组,差异有统计学意义(P<0.05)。试验组术后3个月椎间融合率高于对照组,差异有统计学意义(χ^(2)=4.586,P=0.032);两组术后6个月椎间融合率比较差异无统计学意义(χ^(2)=0.447,P=0.504)。结论颈椎前路椎间盘切除植骨融合术中应用3D打印多孔钛合金椎间融合器可减缓椎体沉降、维持颈椎的生理曲度、加速椎间融合,疗效满意。 Objective To explore clinical efficacy and safety of 3D print porous titanium alloy intervertebral fusion cage in an-terior cervical discectomy and bone graft fusion for treatment of cervical spondylopathy.Methods A total of 67 patients withcervical spondylopathy who underwent anterior cervical discectomy and bone graft fusion from June 2018 to June 2020 were in-cluded and assigned into experiment group(n=33)and control group(n=34),by a random number table.3D print porous titani-um alloy lumbar fusion cage was implanted in experiment group while PEEK lumbar fusion cage was implanted in control group.Duration of surgery,intraoperative blood loss,cervical JOA score,cervical sagittal parameters(height of fusion segment,C_(2-7)Cobb angle),intervertebral fusion were compared between two groups.Results All surgeries were successful performed andall incisions healed primarily.Mean time of follow up was 14.6±4.2 months(12-20 months).No significant complications oc-curred.Between the two groups,duration of surgery,intraoperative blood loss and cervical JOA score 6 moths after surgery werenot significantly different(P>0.05);Height of fusion segment and C_(2-7)Cobb angle one week after surgery were not significantlydifferent(P>0.05).Both height of fusion segment and C_(2-7)Cobb angle 3 months and 6 months after surgery in experiment groupwere significantly higher than those in control group(P<0.05).Intervertebral fusion rate 3 months after surgery in experimentgroup was significantly higher than that in control group(χ^(2)=4.586,P=0.032).Intervertebral fusion rate 6 months after surgeryin experiment group was not significantly different from that in control group(χ^(2)=0.447,P=0.504).Conclusion Use of 3Dcage in anterior cervical discectomy and bone graft fusion can slow down subsidence of the vertebral body,maintain physiologi-cal curvature of cervical spine and accelerate intervertebral fusion.The efficacy is satisfactory.
