摘要
目的对比体素内不相干运动扩散加权成像(IVIM-DWI)及钆塞酸二钠(Gd-EOB-DTPA)增强T1mapping对肝脏储备功能评估的诊断效能。资料与方法回顾性纳入2017年11月—2021年11月云南省第一人民医院1周内完成Gd-EOB-DTPA增强MRI、IVIM-DWI及吲哚菁绿(ICG)滞留试验的乙型肝炎背景患者82例,根据ICG 15 min滞留率(ICG R15)将患者分为3组(<10%49例,10%~30%21例,≥30%12例)。比较3组间T1弛豫时间(T1_(pre)、T1_(20 min))、肝细胞增强分数(HEF)、表观扩散系数(ADC)、真性扩散系数(D)、灌注系数(D*)和灌注分数(f)的差别,采用受试者工作特征曲线评估上述参数对肝储备功能的诊断效能,采用DeLong检验比较不同参数的诊断效能。结果T1mapping参数(T1_(pre)、T1_(20 min)、HEF)在3组中差异均有统计学意义(F=14.290、38.343、42.978,均P<0.01),而IVIM-DWI参数中仅ADC、f、D值差异有统计学意义(F=21.687、24.040、17.949,均P<0.05)。HEF的曲线下面积大于ADC与f值,但ADC、f值的曲线下面积大于T1pre,HEF与ADC、f值的曲线下面积差异无统计学意义(均P>0.05)。结论IVIM-DWI对肝储备功能的评估有一定价值,其诊断效能与T1mapping相近。
Purpose To compare the diagnostic efficiency of intravoxel incoherent motion diffusion-weighted imaging(IVIM-DWI)and gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced T1mapping for assessment of hepatic functional reserve.Materials and Methods Eighty-two patients with hepatitis B background who completed Gd-EOB-DTPA enhanced MRI scans,IVIM-DWI scans and indocyanine green(ICG)retention test within a week in the First People’s Hospital of Yunnan Province from November2017 to November 2021 were retrospectively included.All patients were divided into three groups according to ICG 15 min clearance rate(ICG R15),including 49 patients with ICG R15<10%,21 patients with ICG R15 between 10%and 30%,and 12 patients with ICG R15≥30%.The T1 relaxation time(T1_(pre),T1_(20 min)),hepatocyte uptake fraction(HEF),apparent diffusion coefficient(ADC),true diffusion coefficient(D),perfusion coefficient(D*)and perfusion fraction(f)values were compared among three groups.The diagnostic efficiency of the parameters mentioned above was assessed via the receiver operating characteristic curve.The DeLong’s test was used to compare the diagnostic efficiency of different parameters.Results There were significant differences in T1mapping parameters(T1_(pre),T1_(20 min),HEF)among three groups(F=14.290,38.343,42.978,respectively;all P<0.01),while only ADC,f and D of IVIM-DWI parameters had statistical differences among three groups(F=21.687,24.040,17.949,respectively;all P<0.05).In pairwise comparison among three groups with each other,the area under curve(AUC)of HEF was significantly higher than those obtained by IVIM,but the AUC of ADC and f were significantly higher than T1 pre.The AUC of HEF was not significantly different from that of the parameters ADC and f(all P>0.05).Conclusion IVIM-DWI has a certain value in evaluating liver reserve function with similar evaluation efficiency of T1mapping.
