摘要
目的探讨中药熏洗配合功能锻炼贯序治疗对Plion骨折术后损伤组织及踝关节功能恢复的影响。方法选择2018年1月至2021年12月在河南省中医院就诊的116例Pilon骨折手术后患者作为研究对象,按照随机数字表法分为试验组和对照组,试验组58例,对照组58例。试验组患者接受中药熏洗配合踝关节功能锻炼的贯序治疗,对照组仅接受踝关节功能锻炼治疗。按方案治疗6个月,观察两组患者临床治疗总有效率、优良率和踝关节疗效,观察治疗前后踝关节周径变化水平即损伤组织恢复情况、疼痛评分,观察中药使用的安全性。结果最终101例患者完成研究,试验组50例,对照组51例。两组患者临床治疗总有效率为100.00%,试验组优良率为94.00%,对照组为86.27%。试验组总体治疗效果优于对照组(P=0.007)。试验组改良Mazur踝关节功能疗效评价评分优于对照组(P<0.05),治疗6个月后试验组踝关节周径恢复优于对照组(P<0.05),VAS疼痛评分优于对照组(P<0.05)。使用中药熏洗中有4例发生局部接触性皮炎,停药和涂抹抗过敏药物后症状缓解。结论中药熏洗配合功能锻炼采取贯序治疗的方案对Pilon骨折术后踝关节功能恢复产生良好的促进作用,组方较为安全,值得临床应用。
Objective To investigate the effect of sequential treatment of traditional Chinese medicine fumigation and washing with functional exercise sequential therapy on the recovery of injured tissue and ankle joint function after Plion fracture.Methods A total of 116 patients with Pilon fractures who were treated in Henan Provincial Hospital of Traditional Chinese Medicine from January 2018 to December 2021 were selected as the research objects,and they were divided into the experimental group and the control group according to the random number table method,with 58 cases in the experimental group and 58 cases in the control group.The patients in the experimental group were treated with traditional Chinese medicine fumigation and washing combined with ankle joint functional exercise.The control group was only given ankle joint functional exercise treatment.After 6 months of treatment according to the plan,the total effective rate,excellent and good rate of clinical treatment and evaluation of ankle joint efficacy were observed in the two groups,the change level of ankle joint circumference before and after treatment,the recovery of damaged tissue,the pain score and the safety of traditional Chinese medicine were observed.Results Finally,101 patients completed the study,50 in the experimental group and 51 in the control group.The total effective rate of the two groups was 100.00%.The excellent and good rates of the experimental group and the control group were 94.00%and 86.27%,respectively.The overall treatment effect of the test group was better than that of the control group(P=0.007).The experimental group was better than the control group with the modified Mazur ankle function evaluation score(P<0.05),After 6 months of treatment,the recovery of ankle circumference in the experimental group was better than that in the control group(P<0.05).The VAS pain score in the experimental group was better than that in the control group(P<0.05).Four cases of local contact dermatitis occurred in the use of traditional Chinese medicine fumigation and washing,and the symptoms were relieved after drug withdrawal and application of antiallergic drugs.Conclusion The treatment of traditional Chinese medicine fumigation and washing with functional exercise sequential therapy has a good promoting effect on the recovery of ankle joint function after Pilon fracture.The prescription is relatively safe and worthy of clinical application.
