摘要
目的评价急性缺血性脑血管病实施磁共振弥散加权成像的诊断价值,为临床诊断急性缺血性脑血管病的方式选择方面提供依据。方法选取本院2020年1月-2021年1月收治110例患者参与研究。所有研究对象均给予CT检查、MRI检查。以DSA检查结果作为确诊患者病情的最终结果,分析CT诊断、MRI诊断急性缺血性脑血管病的灵敏度、特异度、阳性预测值、阴性预测值、准确率。之后选取DSA确诊的患者再次进行研究,观察不同检查方式诊断患者的结果和MRI不同序列诊断病灶的结果。结果CT诊断急性缺血性脑血管病的灵敏度为59.00%,特异度为30.00%,阳性预测值为89.39%,阴性预测值为6.38%,准确率为56.36%;MRI诊断急性缺血性脑血管病的灵敏度为95.00%,特异度为80.00%,阳性预测值为97.94%,阴性预测值为61.54%,准确率为93.64%。与CT诊断进行统计对比发现,MRI诊断阴性率更低;MRI诊断短暂性脑缺血发作率、可逆性神经功能缺损率、腔隙梗死率、完全性卒中率更高,差异有统计学意义(P<0.05)。与常规序列诊断进行统计对比发现,弥散加权成像诊断急性脑梗死率、动脉瘤率、蛛网膜下腔出血率、脑微出血率更高,差异有统计学意义(P<0.05)。结论急性缺血性脑血管病实施磁共振弥散加权成像的灵敏度、特异度、阳性预测值、阴性预测值、准确率均较高,能得到比较准确的诊断结果,即使是急性缺血性脑血管病的不同类型和不同病灶,都能得到比较高的诊断准确率。
Objective To evaluate diagnostic value of diffusion-weighted magnetic resonance imaging(DWI)for acute ischemic cerebrovascular disease(AICVD),to provide evidence for clinical diagnosis of AICVD.Methods The paper chose 110 patients examined with CT and MRI in our hospital from January 2020 to January 2021 for study.Based on DSA examination result as final result of confirmed patient's condition,sensitivity,specificity,positive predictive value,negative predictive value and accuracy of CT diagnosis and MRI diagnosis of acute ischemic cerebrovascular disease was analyzed.After that,patients confirmed by DSA were selected for further study,rsults of patients diagnosed by different examination methods and lesions diagnosed by different MRI sequences were observed.Results Sensitivity,specificity,positive predictive value,negative predictive value and accuracy of CT in diagnosing acute ischemic cerebrovascular disease was 59.00%,30.00%,89.39%,6.38%and 56.36%respectively;sensitivity,specificity,positive predictive value,negative predictive value and accuracy of MRI in diagnosing acute ischemic cerebrovascular disease was 95.00%,80.00%,97.94%,61.54%and 93.64%respectively.Negative rate of MRI diagnosis was lower than CT diagnosis;rate of transient ischemic attack,reversible neural function defect,lacunar infarction and complete stroke diagnosed by MRI were higher,difference was statistically significant(P<0.05).Statistical comparison with routine sequence diagnosis showed,rate of acute cerebral infarction,aneurysm,subarachnoid hemorrhage and cerebral microbleeding diagnosed by diffusion weighted imaging was higher,difference was statistically significant(all P<0.05).Conclusion Diffusion-weighted magnetic resonance imaging can achieve high sensitivity,specificity,positive predictive value,negative predictive value and accuracy rate for acute ischemic cerebrovascular disease,with relatively accurate diagnostic results,even for different types and lesions of acute ischemic cerebrovascular disease.
