摘要
目的探讨高频超声引导下肩关节腔内药物注射联合穴位按摩治疗肩周炎患者68例疗效观察。方法回顾性分析解放军联勤保障部队第九○四医院2017年1月至2020年12月收治的肩周炎患者68例的临床资料,在高频超声引导下肩关节腔内注射玻璃酸钠、倍他米松、维生素B12及生长因子等混合药液,并联合肩关节周围穴位按摩,观察比较治疗前、后的疗效。结果患者肩关节腔内药物注射治疗联合其周围穴位按摩治疗后1个月总有效率52.94%(36/68),治疗后3个月总有效率70.59%(48/68),治疗后6个月总有效率94.12%(64/68),患者治疗后较治疗前疗效确切(P<0.05)。患者治疗后6个月,肩关节屈曲(131.4±5.3°)、内收(20.1±3.2°)、外旋活动度(30.2±5.6°)较治疗前肩关节屈曲(82.2±8.7°)、内收(10.8±2.3°)、外旋活动度(18.3±4.7°)显著提高(P<0.05);Constant-Murley评分治疗后(52.8±3.9分)较治疗前(32.3±4.2分)提高(P<0.05);生活质量综合评定问卷-74(GQOL-74,成人用)评分治疗后较治疗前提高(P<0.05);视觉模糊评分(VAS)较治疗后(2.1±1.3)较治疗前(8.6±1.5)降低(P<0.05)。结论高频超声引导下肩关节腔内药物注射联合周围穴位按摩治疗肩周炎,疗效明显,长期愈后好,患者生活质量明显提高,方法安全,是中西医联合治疗疾病的典范,值得临床推广。
Objective This study aimsto investigate the therapeutic effect of High Frequency Ultrasound-guided intra-articular drug injection and acupoint massage on 68 cases of shoulder periarthritis.Methods A total of 68 patients with periarthritis of shoulder who were admitted to Wuxi 904 Hospital of the Joint Logistics Department of the people’s Liberation Army from January 2017 to December 2020 were enrolled.Under the guidance of high-frequency ultrasound,the mixed liquid of sodium hyaluronate,betamethasone,vitamin B12 and growth factor were injected into the shoulder joint cavity,and combined with acupoint massage around the shoulder joint.The effects before and after treatment were observed and compared.Results The total effective rate was 52.94%(36/68)in the first month after treatment,70.59%(48/68)in the third month after treatment,and 94.12%(64/68)in the sixth month after treatment.The curative effect of the patients after treatment was better than that before treatment(P<0.05).Six months after treatment,the patient’s shoulder joint flexion(131.4±5.3°),Adduction(20.1±3.2°),External rotation activity(30.2±5.6°)were significantly increased than the shoulder flexion before treatment(82.2±8.7°),Adduction(10.8±2.3°),External rotation mobility(18.3±4.7°)(P<0.05).The constant Murley score after treatment(52.8±3.9 points)was higher than that before treatment(32.3±4.2 points)(P<0.05).The score of gqol-74(comprehensive quality of life assessment questionnaire-74,for adults)after treatment was higher than that before treatment(P<0.05).Visual acuity score(VAS)was lower than that after treatment(2.1±1.3)and before treatment(8.6±1.5)(P<0.05).Conclusion Under the guidance of high-frequency ultrasound,intra-articular drug injection combined with peripheral acupoint massage in the treatment of scapulohumeral periarthritis has obvious curative effect,better long-term recovery,significantly improved patient’s quality of life,and excellent security.It is a model of combined treatment of Chinese and Western medicine,and it is worthy of clinical promotion.
