摘要
目的对动态心电图(dynamic electrocardiogram,DCG)联合平板运动试验(treadmill exercise test,TMT)评估室性早搏(ventricular premature contraction,VPC)患儿的价值进行分析。方法将2018年1月至2021年1月在郑州大学附属儿童医院就诊的88例室性早搏患儿纳入到本次研究中,经过检查后将其分为A、B两组,A组为非器质性心脏病组(n=42),B组为器质性心脏病组(n=46)。两组患儿均接受DCG、TMT检查,对比两组的检查结果,日间型和夜间型患儿的运动风险,以及DCG、TMT对器质性室性早搏的检出率。结果(1)采用DCG检查方法,均匀散在分布的VPC、无规律的VPC、成对VPC、短阵型VPC检测占比较高(P<0.05)。(2)采用TMT检查方法成对VPC、短阵型VPC、多元性VPC、ST段改变所占比例较高(P<0.05)。(3)对比日间型和夜间型患儿运动风险,日间高发型VPC更容易出现运动异常(P<0.05)。(4)两种检查方法对器质性VPC检查结果所表现出的差异并无统计学意义(P>0.05)。结论DCG和TMT两种方法都具有自身的优势,联用可发挥出更好的效果。将DCG联合TMT应用在室性早搏患儿的运动风险评估中,可对患儿的心律失常情况进行有效的监测,为医生临床评估VPC患儿的运动风险提供一定的诊断参考,提升评估的科学性和有效性,值得推广与应用。
Objective To analyze the value of dynamic electrocardiogram(DCG)combined with treadmill exercise test(TMT)in evaluating children with premature ventricular beat.Methods A total of 88 children with ventricular premature contraction(VPC)who received medical treatment in our hospital from January 2018 to January 2021 were enrolled in this study.After examination,they were divided into two groups:group A,non-organic heart disease group(n=42),and group B,organic heart disease group(n=46).DCG and TMT examinations were performed in both groups,and the results of the examinations were compared between the two groups,as well as the risk of exercise and the detection rate of DCG and TMT on organic premature ventricular beat.Results(1)The DCG method was used to detect evenly scattered VPC,irregular VPC,paired VPC and short-formation VPC(P<0.05).(2)The changes of VPC,short-formation VPC,diversified VPC and ST segment by TMT examination accounted for a higher proportion(P<0.05).(3)By comparing the exercise risk of diurnal and nocturnal children,VPC with high daytime high incidence type was more prone to abnormal movement(P<0.05).(4)There was no statistically significant difference between the two methods in organic VPC examination results(P>0.05).Conclusion Indicating that both methods have their own advantages and can play a better effect when used together the application of DCG+TMT in the exercise risk assessment of children with premature ventricular beat can effectively monitor the arrhythmia of children,provide sufficient diagnostic reference for doctors to evaluate the exercise risk of children with VPC,improve the scientific and effectiveness of the assessment,and is worthy of promotion and application.
