摘要
【目的】观察五味消毒饮加减对全髋关节置换术后患者早期疗效的影响。【方法】将60例行人工全髋关节置换术的患者随机分为对照组和观察组,每组各30例。对照组术后采取常规西药治疗,观察组在对照组的基础上加用五味消毒饮加减治疗,疗程为1周。观察2组患者治疗前和治疗1周后的大腿周径差、小腿周径差、疼痛视觉模拟量表(VAS)评分、髋关节功能Harris评分的变化情况,并记录2组患者的不良事件发生情况。【结果】(1)治疗后,2组患者的大腿周径差和小腿周径差均较治疗前明显下降(P<0.05),且观察组对大腿周径差的下降作用明显优于对照组,差异有统计学意义(P<0.01),而2组治疗后的小腿周径差比较,差异无统计学意义(P>0.05)。(2)治疗后,2组患者的疼痛VAS评分均较治疗前明显降低(P<0.05),髋关节功能Harris评分均较治疗前明显提高(P<0.05),且观察组对疼痛VAS评分的降低作用和对髋关节功能Harris评分的提高作用均明显优于对照组,差异均有统计学意义(P<0.01)。(3)并发症发生情况方面,观察组1例患者发生下肢深静脉血栓,并发症发生率为3.33%(1/30);对照组1例患者发生术口延迟愈合,并发症发生率亦为3.33%(1/30);2组患者的并发症发生率比较,差异无统计学意义(P>0.05)。【结论】全髋关节置换术后患者应用五味消毒饮加减治疗效果显著,能够有效缓解髋关节术后肿痛,促进髋关节功能恢复。
Objective To observe the influence of modified Wuwei Xiaodu Decoction on the early therapeutic effect in patients after total hip arthroplasty(THA).Methods Sixty patients undergoing THA were randomly divided into control group and observation group,with 30 cases in each group.The control group was treated with conventional western medicine after operation,while the observation group was treated with Wuwei Xiaodu Decoction on the basis of treatment for the control group.The course of treatment lasted for one week.The changes in thigh circumference difference,calf circumference difference,pain visual analogue scale(VAS)scores and Harris scores of hip joint function in the two groups were observed before treatment and after one week of treatment.Meanwhile,the incidence of adverse events in the two groups was recorded.Results(1)After treatment,the thigh circumference difference and calf circumference difference of patients in both groups were significantly decreased compared with those before treatment(P<0.05).The intergroup comparison showed that the effect on decreasing the thigh circumference difference of the observation group was significantly superior to that of the control group,and the difference was statistically significant(P<0.01),but the difference in the calf circumference difference between the two groups after treatment was not statistically significant(P>0.05).(2)After treatment,the pain VAS scores of both groups were significantly decreased and the Harris scores of hip joint function were significantly increased compared with those before treatment(P<0.05).The intergroup comparison showed that the effects on decreasing pain VAS scores and on increasing Harris scores in the observation group were significantly superior to those in the control group,and the differences were statistically significant(P<0.01).(3)In terms of complications,one patient in the observation group suffered from deep vein thrombosis of the lower limb,with the incidence of the complications being 3.33%(1/30);one patient in the control group suffered from delayed healing of the surgical incision,with the incidence of the complications also being 3.33%(1/30).The intergroup comparison showed that the difference in the incidence of the complications between the two groups was not statistically significant(P>0.05).Conclusion Modified Wuwei Xiaodu Decoction exerts significant effect for the treatment of patients after THA,and it is effective on relieving post-operative swelling and pain,and on promoting the recovery of the hip joint function.
