摘要
目的研究MRI联合彩色超声在诊断乳腺癌中的诊断效能。方法选择2018年11月至2020年11月收治的100例疑似乳腺癌患者,分别予以彩色超声、MRI、MRI联合彩色超声检查,将手术病理组织学检查作为金标准,比较彩色超声、MRI、MRI联合彩色超声诊断灵敏度、特异度,Kappa检验手术病理诊断与MRI的一致性,比较彩色超声、MRI、MRI联合彩色超声不同肿块直径乳腺癌检出率,分析超声与MRI影像学特征。结果MRI联合彩色超声诊断灵敏度、特异度均高于彩色超声、MRI(P<0.05)。彩色超声与手术病理组织学的Kappa一致性较差,MRI与手术病理组织学的一致性一般,MRI联合彩色超声与手术组织病理学的Kappa一致性较好。MRI联合彩色超声对≤1 cm肿块检出率高于彩色超声、MRI(P<0.05);MRI联合彩色超声对1~2 cm肿块检出率与彩色超声、MRI比较,差异无统计学意义(P>0.05)。结论MRI联合彩色超声可提高乳腺癌诊断灵敏度、特异度,与手术病理组织学诊断结果相当,诊断价值较高。
Objective To investigate the effects of MRI combined with color Doppler ultrasound in the diagnosis of breast cancer.Methods A total of 100 patients with suspected breast cancer,who were treated from November 2018 to November 2020,underwent color Doppler ultrasound,MRI and combined detection of color Doppler ultrasound and MRI,respectively.The surgical histologic examination was taken as the gold standard.The diagnostic specificity and sensitivity of the three approaches were compared.Moreover,Kappa test was used to determine the consistency between pathological diagnosis and MRI.In addition,the detection rates of breast cancer with different tumor diameter by the three approaches were compared and imaging features of color Doppler ultrasound and MRI were analyed.Results The diagnostic sensitivity(94.79%)and specificity(75.00%)of combined detection of color Doppler ultrasound and MRI were higher than those of color ultrasound(83.33%,25.00%)and MRI(88.54%,50.00%),(P<0.05).The consistency between color Doppler ultrasound,MRI,combined detection of color Doppler ultrasound and MRI and surgical histopathology was bad,moderate and good,respectively.Moreover,the detection rate of≤1cm mass of combined detection was higher those of separate detection(P<0.05).But there was no significant difference in the detection rate of 1~2cm mass between the three approaches(P>0.05).Conclusion MRI combined with color Doppler ultrasound can improve the diagnostic sensitivity and specificity of breast cancer,and is consistent with the results of surgical histopathology,which is of high diagnostic value.
