摘要
目的观察滋肾宁心胶囊治疗肝肾阴虚型围绝经期高血压的临床疗效。方法将80例围绝经期高血压肝肾阴虚型患者按照随机数字表法分为2组,对照组40例予替米沙坦片治疗,治疗组40例在对照组治疗基础上加滋肾宁心胶囊治疗,2组均连续治疗12周。比较2组治疗前后诊室血压[收缩压(SBP)、舒张压(DBP)]、动态血压[24 h平均收缩压(24 hmSBP)、24 h平均舒张压(24 hmDBP)、日间平均收缩压(dmSBP)、日间平均舒张压(dmDBP)、夜间平均收缩压(nmSBP)、夜间平均舒张压(nmDBP)]和血压变异系数(CV)(24 hSBP-CV、dSBP-CV、nSBP-CV、24 hDBP-CV、dDBP-CV、nDBP-CV)、动态心率[24 h平均心率(24 hmHR)、日间平均心率(dmHR)、夜间平均心率(nmHR)]、中医证候评分、广泛性焦虑量表(GAD-7)评分、匹兹堡睡眠质量指数(PSQI)评分。结果治疗后,2组诊室SBP、DBP及动态血压24 hmSBP、24 hmDBP、dmSBP、dmDBP、nmSBP、nmDBP,24 hSBP-CV、dSBP-CV、nSBP-CV、24 hDBP-CV、dDBP-CV、nDBP-CV,24 hmHR、dmHR、nmHR均较本组治疗前下降(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后,2组除耳鸣、健忘外,其余中医证候评分均较本组治疗前下降(P<0.05)。治疗后,治疗组除耳鸣、健忘外,其余中医证候评分均低于对照组(P<0.05)。治疗后,2组PSQI各单项评分和总分以及GAD-7各单项评分和总分均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。结论滋肾宁心胶囊不仅能降低肝肾阴虚型围绝经期高血压患者诊室血压、动态血压的各时段平均血压,降低CV、HR,减少血压波动,还可降低PSQI评分和GAD-7评分,改善患者睡眠质量和焦虑状态,并改善围绝经期相关中医证候。
Objective To observe the clinical efficacy of Zishen Ningxin Capsule for perimenopausal hypertension(liver and kidney yin deficiend type).Methods Totally 80 patients with perimenopausal hypertension(liver and kideyrin deficiency type)were randomly assigned into treatment group(n=40)and control group(n=40).All patients were managed by telmisartan tablet,and those in observation group were additionally treated with Zishen Ningxin Capsule.A 12-week treatment was performed to compare clinic blood pressure(CBP)(systolic blood pressure[SBP],diastolic blood pressure[DBP]),ambulatory blood pressure(ABP)(24-h mean SBP[24 hmSBP],24-h mean DBP[24 hmDBP],daytime mean SBP[dmSBP],daytime mean DBP[dmDBP],nocturnal mean SBP[nmSBP],nocturnal mean DBP[nmDBP]),blood pressure coefficient of variation(CV)(24 hSBP-CV,dSBP-CV,nSBP-CV,24 hDBP-CV,dDBP-CV,nDBP-CV),dynamic heart rate(HR)(24-h mean of HR[24 hmHR],daytime mean HR[dmHR],nocturnal mean HR[nmHR]),traditional Chinese medicine(TCM)syndrome score,the 7-item Generalized Anxiety Disorder Questionnaire(GAD-7),Pittsburgh Sleep Quality Index(PSQI).Results After treatment,CBP,ABP,blood pressure CV,dynamic HR in groups were decreased(P<0.05),treatment group was more prevalent(P<0.05);apart from tinnitus,forgetfulness,the other TCM syndrome scores in groups were decreased(P<0.05),which were decreased in treatment group in comparison with control group(P<0.05);PSQI scores and GAD-7 scores in groups were decreased(P<0.05),the reduction in treatment group was more common(P<0.05).Conclusion For patients with perimenopausal hypertension(liver and kidney yin deficiency type),Zishen Ningxin Capsule can relieve the average blood pressure of each period of CBP and ABP,reduce CV,HR,decrease blood pressure fluctuation,PSQI scores and GAD-7 scores,improve sleep quality,anxiety status,and TCM syndromes.
