摘要
目的探究温阳通络方联合加巴喷丁对糖尿病周围神经病变患者超敏C反应蛋白水平和蛋白激酶C活性的影响。方法选取2019年5月至2020年5月温州市中医院收治的糖尿病周围神经病变患者60例,通过随机数字表法分为单药组与联合组,每组30例。两组患者均行常规治疗,单药组使用加巴喷丁进行治疗,联合组在单药组基础上给予温阳通络方进行治疗。比较两组患者治疗前后的空腹血糖(fasting blood glucose,FBG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、中医证候评分、感觉神经传导速度(sensory nerve conduction velocity,SNCV)、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、血清一氧化氮(serum nitric oxide,NO),检测患者治疗前后超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)水平、蛋白激酶C(protein kinase C,PKC)活性,对比两组治疗有效率。结果治疗后,联合组的FBG、HbA1c、主症评分及中医证候评分均低于单药组,差异均有统计学意义(P<0.05)。治疗后两组SNCV均有改善,但与治疗前比较,差异无统计学意义(P>0.05)。治疗后,联合组的TNF-α、PKC、hs-CRP水平低于单药组,NO水平高于单药组,差异均有统计学意义(P<0.05)。联合组的治疗总有效率高于单药组,差异有统计学意义(P<0.05)。结论对糖尿病周围神经病变患者使用温阳通络方联合加巴喷丁进行治疗,能够降低患者血糖水平,改善患者hs-CRP水平及PKC活性,降低机体炎症程度,治疗效果显著。
Objective To explore the effect of Wenyang Tongluo Formula combined with gabapentin on the level of hypersensitive C-reactive protein(hs-CRP)and protein kinase C(PKC)activity in patients with diabetic peripheral neuropathy.Methods A total of 60 patients with diabetic peripheral neuropathy who were admitted to Wenzhou Hospital of Traditional Chinese Medicine from May 2019 to May 2020 were selected and randomly divided into the monotherapy group and the combined treatment group by random number table method,with 30 cases in each group.Both groups were given routine treatment,the monotherapy group was treated with gabapentin,and the combined treatment group was treated with Wenyang Tongluo Formula on the basis of the monotherapy group.The fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),traditional Chinese medicine syndrome score,sensory nerve conduction velocity(SNCV),tumor necrosis factor-α(TNF-α),serum nitric oxide(NO)in two groups before and after treatment were observed and compared,detected the level of high sensitivity C-reactive protein(hs-CRP)and protein kinase C(PKC)activity in patients before and after treatment,and compared the treatment efficiency between the two groups.Results After treatment,the levels of FBG and HbA1c,the scores of main symptoms and traditional Chinese medicine syndromes in the combination therapy group were lower than those in the monotherapy group,and the differences were statistically significant(P<0.05).SNCV improved in both groups after treatment,but the difference was not statistically significant when compared to pre-treatment(P>0.05).After treatment,the levels of TNF-α,PKC and hs-CRP in the combined therapy group were lower and the NO value was higher than those in the monotherapy group,and the differences were statistically significant(P<0.05).And compared with the patients in the monotherapy group,the total effective rate of the patients in the combination therapy group was statistically higher(P<0.05).Conclusion Wenyang Tongluo Formula combined with gabapentin in the treatment of diabetic peripheral neuropathy can reduce the blood glucose level of patients,improve the hs-CRP level and PKC activity of patients,reduce the degree of inflammation,and the treatment effect is significant.
