摘要
目的分析高龄初产妇剖宫产术后社会支持与纯母乳喂养的关系。方法选取2020年1月至2021年10月信阳市中心医院收治的215例高龄初产妇为研究对象,均接受剖宫产分娩,通过问卷了解其术后社会支持评定量表(SSRS)评分、纯母乳喂养率,并统计其临床资料,分析影响纯母乳喂养的因素。结果高龄初产妇剖宫产术后SSRS评分为(25.67±3.19)分,处于一般水平;130例高龄初产妇给予纯母乳喂养,占60.47%;logistic回归分析结果显示,开奶时间>24 h、住院期间哺乳困难、未参加母乳喂养教育、产后无访视服务、丈夫不支持母乳喂养、社会支持较少、乳头凹陷或扁平均为高龄初产妇剖宫产术后纯母乳喂养的独立危险因素(P<0.05)。结论高龄初产妇剖宫产术后社会支持水平一般,且可对纯母乳喂养率产生影响。为提高纯母乳喂养率,临床不仅需关注高龄初产妇剖宫产术后社会支持水平,还需加强对开奶时间、产后访视服务、母乳喂养教育等的关注。
Objective To analyze the relationship between social support and exclusive breastfeeding after cesarean delivery in elderly primigravida.Methods A total of 215 cases of elderly primigravida who were treated in Xinyang Central Hospital from January 2020 to October 2021 were selected as subjects,all received cesarean delivery.And their postoperative social support rating scale(SSRS)scores and exclusive breastfeeding rate were obtained by questionnaire,and clinical data were counted to analyze the factors affecting exclusive breastfeeding.Results The SSRS score after cesarean section in elderly primigravida was(25.67±3.19),which was at an average level.And 130 elderly primigravida gave exclusive breastfeeding,accounting for 60.47%.Logistic regression equation analysis showed that time to breast initiation>24 h,difficulty in breastfeeding during hospitalization,not attending breastfeeding education,no postpartum visiting service,husband not supporting breastfeeding,less social support and nipple depression or flattening on average were independent risk factors for exclusive breastfeeding after cesarean section in older primigravida(P<0.05).Conclusion The level of social support after cesarean section in elderly primigravida is average and can have an impact on the rate of exclusive breastfeeding.In order to improve the exclusive breastfeeding rate,clinical attention should be paid not only to elderly primigravida postoperative social support level,but also to the timing of milk initiation,postpartum visitation service and breastfeeding education.
