摘要
目的:探讨失能老年人居家照顾者对医养结合服务需求调查及影响因素。方法:于2021年10月~2021年12月采取便利抽样法对北京市东城区496例失能老年人居家照顾者进行上门问卷调查,问卷内容包括居家照顾者的一般资料和医养结合服务需求,并采用Logistic回归分析其影响因素。结果:居家照顾者CBI评分为(31.09±8.74),SSRS评分为(30.36±6.63)分,SDS评分为(46.24±6.12)分;496例居家照顾者中,146例有医养结合服务需求,需求率为29.44%。其中99例(67.81%)有医疗护理需求,90例(61.64%)有健康体检需求,51例(34.93%)有健康教育需求,66例(45.21%)有生活照料需求,32例(21.92%)有精神慰藉需求,38例(26.03%)有社会参与需求;采用多因素Logistic分析,家庭月收入≥5000元、照顾时间≥3年、失能老人Barthel指数<60分、失能老人慢性病数≥2个、CBI评分≥40分、SSRS评分<33分、SDS评分≥50分均为影响失能老年人居家照顾者对医养结合服务需求的独立影响因素。结论:家庭月收入≥5000元、照顾时间≥3年、失能老人Barthel指数<60分、失能老人慢性病数≥2个、CBI评分≥40分、SSRS评分<33分、SDS评分≥50分是失能老年人居家照顾者对医养结合服务需求的独立影响因素,医养结合护理人员应加强宣传教育工作,改善失能老人的健康水平。
Objective To explore the investigation and influencing factors of medical-care service demands in home caregivers of the disabled elderly.Methods According to convenience sampling method,home caregivers for the 496 disabled elderly in Dongcheng District,Beijing,were investigated by door-to-door questionnaire between October and December 2021.The content of questionnaire included general data of home caregivers and medical-care service demands.And the influencing factors were analyzed by logistic regression analysis.Results The scores of CBI,SSRS and SDS in home caregivers were (31.09±8.74) points,(30.36±6.63) points and (46.24±6.12) points,respectively.Among the 496 home caregivers,there were 146 cases (29.44%) with medical-care service demands,including 99 cases (67.81%) with medical nursing demands,90 cases (61.64%) with health physical examination demands,51 cases (34.93%) with health education demands,66 cases (45.21%) with life care demands,32 cases (21.92%) with spiritual consolation demands and 38 cases (26.03%) with social participation demands.Multivariate Logistic analysis showed that family monthly income ≥5000 yuan,care time ≥3 years,Barthel index in the disabled elderly <60 points,number of chronic diseases ≥2,CBI score ≥40 points,SSRS score <33 points and SDS score ≥50 points were all independent influencing factors of medical-care service demands in home caregivers of the disabled elderly.Conclusion The family monthly income ≥5000 yuan,care time ≥3 years,Barthel index in the disabled elderly <60 points,number of chronic diseases ≥2,CBI score ≥40 points,SSRS score <33 points and SDS score ≥50 points are all independent influencing factors of medical-care service demands in home caregivers of the disabled elderly.The nursing staffs should strengthen publicity and education to improve health level of the disabled elderly.
