摘要
目的探究根管治疗联合牙周翻瓣术对牙周牙髓联合病变患者牙周指标和龈沟液炎症因子的影响。方法选取2021年1~12月浙江大学医学院附属第二医院临平院区收治的76例牙周牙髓联合病变患者,根据随机数字表法将其分为对照组和观察组,每组各38例。对照组患者采用根管治疗,观察组患者采用根管治疗联合牙周翻瓣术。比较两组患者的治疗疗效、牙周指标[牙周袋探诊深度(probing pocket depth,PD)、探诊出血指数(bleeding index,BI)、牙龈指数(gingival index,GI)、菌斑指数(plaque index,PLI)、牙齿松动度(tooth mobility degree,TM)]、龈沟液炎症因子[肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、基质金属蛋白酶-8(matrix metalloproteinase-8,MMP-8)]及细菌感染情况。结果观察组患者的治疗总有效率显著高于对照组(94.74%vs 78.95%,P<0.05)。治疗后,两组患者的PD、BI、GI、PLI、TM、TNF-α、IL-6、MMP-8均显著低于本组治疗前(P<0.05),且观察组患者的PD、BI、GI、PLI、TM、TNF-α、IL-6、MMP-8均显著低于对照组(P<0.05)。治疗后观察组患者的细菌感染率显著低于对照组(5.26%vs 23.68%,P<0.05),感染清除率显著高于对照组(89.47%vs 68.42%,P<0.05)。结论根管治疗联合牙周翻瓣术可明显改善牙周牙髓联合病变患者的牙周指标,减轻炎症反应,降低患者的细菌感染率,临床疗效较好。
Objective To investigate the effect of root canal therapy combined with periodontal flap surgery on periodontal indexes and inflammatory factors in gingival crevicular fluid in patients with periodontic endodontic lesion.Methods A total of 76 patients with periodontic endodontic lesion admitted to Linping District,the Second Affiliated Hospital Zhejiang University School of Medical,from January to December 2021 were selected and divided into control group and observation group according to the random number table method,38 cases in each group.The patients in control group received root canal therapy,and the patients in observation group received root canal therapy combined with periodontal flap surgery.The curative effect,periodontal index[probing pocket depth(PD),bleeding index(BI),gingival index(GI),plaque index(PLI),tooth mobility degree(TM)],gingival crevicular fluid inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),matrix metalloproteinase-8(MMP-8)]level and bacterial infection were compared between the two groups.Results The total effective rate of observation group was significantly higher than that of control group(94.74%vs 78.95%,P<0.05).After treatment,PD,BI,GI,PLI,TM,TNF-α,IL-6 and MMP-8 in two groups were significantly lower than before(P<0.05),and PD,BI,GI,PLI,TM,TNF-α,IL-6 and MMP-8 in observation group were significantly lower than those in control group(P<0.05).After treatment,the bacterial infection rate in observation group was significantly lower than that in control group(5.26%vs 23.68%,P<0.05),and the infection clearance rate in observation group was significantly higher than that in control group(89.47%vs 68.42%,P<0.05).Conclusion Root canal therapy combined with periodontal flap surgery can significantly improve the periodontal indexes of patients with periodontic endodontic lesion,reduce the inflammatory response,and reduce the bacterial infection rate of patients,with good curative effect.
