摘要
目的探讨彩色多普勒超声对乳腺癌患者放疗期间心功能受损程度的评估价值。方法选取245例乳腺癌患者,均接受放疗,放疗前后给予彩色多普勒超声检查,比较放疗前后心功能指标[舒张早期心室充盈速度最大值(E)、短轴缩短率(FS)、左室射血分数(LVEF)]、血流分级、超声特征、收缩期峰值血流速度(PSV)、阻力指数(RI)、肿瘤体积。结果放疗后,乳腺癌患者的E、FS、LVEF均明显低于放疗前,差异均有统计学意义(P﹤0.01)。放疗后,乳腺癌患者的血流分级明显优于放疗前,差异有统计学意义(P﹤0.01)。放疗后,乳腺癌患者后方回声减弱率、弱内部回声率、边界清晰率、形态欠规整率均明显高于放疗前,差异均有统计学意义(P﹤0.01)。放疗后,乳腺癌患者的PSV、RI均明显低于放疗前,肿瘤体积明显小于放疗前,差异均有统计学意义(P﹤0.01)。结论乳腺癌患者放疗期间通过彩色多普勒超声检查可以准确评估患者的心脏功能受损程度,同时可提供病灶超声特征、血流参数等信息,辅助临床针对性治疗。
Objective To explore the value of color Doppler ultrasound in evaluating the degree of cardiac function damage in patients with breast cancer during radiotherapy.Method A total of 245 patients with breast cancer were selected,who received radiotherapy.Before and after radiotherapy,they were examined by color Doppler ultrasound.Cardiac function indexes[maximum early diastolic ventricular filling velocity(E),fraction shortening(FS),left ventricular ejection fraction(LVEF)],blood flow classification,ultrasonic characteristics,peak systolic velocity(PSV),resistance index(RI),and tumor volume before and after radiotherapy were compared.Result After radiotherapy,the E,FS and LVEF were lower than those before radiotherapy,the differences were statistically significant(P<0.01).After radiotherapy,the blood flow grading of breast cancer patients was significantly better than that before radiotherapy,the difference was statistically significant(P<0.01).After radiotherapy,the posterior echo attenuation rate,weak internal echo rate,clear border rate,and irregular shape rate of breast cancer patients were significantly higher than those before radiotherapy,the differences were statistically significant(P<0.01).After radiotherapy,the PSV and RI of breast cancer patients were significantly lower than those before radiotherapy,and the tumor volume was significantly smaller than that before radiotherapy,the differences were statistically significant(P<0.01).Conclusion Color Doppler ultrasonography can accurately evaluate the degree of cardiac function impairment during radiotherapy of patients with breast cancer,and can provide ultrasound features and blood flow of lesions to support clinical treatment.
